Our Older Adults Community Mental Health Team provides a range of services including a Memory Service, Care Co-ordination, Social Care Assessment, Mental Health Assessment & Crisis Intervention, Enhanced Duty Response and Step up care.

We operate an innovative and integrated assessment for our patients with the aim of providing timely assessment and diagnosis following the first appointment, and care close to the persons home. We work closely with our partners in Bucks County council .

You will be expected to deliver and maintain high quality and person centred care for our patients. Therefore, the successful candidate will need to be a highly skilled and motivated practitioner who has extensive experience of mental health services and can demonstrate the ability to teach and lead others. A caring and committed attitude with a strong understanding of the needs of older adults with dementia and a variety of mental health needs is essential. A good working knowledge of safeguarding and process to manage risk is required.
