Main duties of the job

Work closely with all of the paediatricians within the Trust and referring consultants from tertiary centres to provide a comprehensive, high quality service for children who are under the care of a paediatrician at Medway NHS Foundation Trust.

Job responsibilities

Be a resource within the Medway NHS Foundation Trust and other agencies across Medway and Swale who are involved in caring for this group of children.

Take part in liaison, teaching and support to all agencies involved inmanaging children with chronic medical conditions.

To have experience or willing to undertake the cannulation & phlebotomy course

Be responsible for the provision of competent community nursing support, to meet individual needs of the children, their families/carers.

Contribute to the quality and development of nursing care in Paediatrics by participating in and organizing teaching programmes, whilst working inpartnership with the children and their families and or carers.

To assess childrens needs and develop and evaluate care plans for anidentified patient group and maintain accurate, contemporaneous recordspreserving patient confidentiality, submitting reports and activity data asrequested.

To plan and provide specialist nursing care for children who are patients of Medway NHS Foundation Trust in community settings, regularly assessing the care and support that is provided within the home/community.

To organise home visits for children with chronic medical conditions andprovide support for children and their families.

To encourage parental participation in the care of their child by providingspecialist support and practical guidance to these families.

To provide a supportive and guiding role for families with a terminally ill child at home, participating in a 24-hour on-call rota, if necessary, to supportsymptom control management.

Provide and participate in timely training and support to patients, families and staff of outside agencies involved with this patient group.

Promote and facilitate best practice in all aspects of care delivered to thechildren and their families and or carers.

To be a resource in childrens chronic medical conditions, demonstratingskills and expertise to all grades of staff and participating in teaching andassessing programmes.

With all members of the Paediatric Nursing Team establish a good working environment in which all children and families receive a high standard of Family Centred Care maintaining respect and privacy

Critically appraise current practice and identify areas for improvement in the light of current research.

To regularly evaluate, and monitor by using credible systems that the caredelivered to patients is of the highest standard.

Ensure that nursing documentation is complete, legible, accurate andrelevant, therefore complying with the Nursing and Midwifery Council and theLord Laming Enquiry recommendations.

Refer on and have an in depth understanding of Safeguarding Children andYoung People, Child in Need concerns ensuring that local and nationalguidelines including the London Child Protection procedures are adhered to.

To have a knowledge and understanding of incidents reporting andimplementation of Paediatric Risk Management, using the DATIX procedures.

Facilitate and supervise a supportive learning environment for students and other nursing staff within the Trust.

Encourage innovation by all members of the team and to personally be up to date in all matters relating to nursing practice

In addition, applicants should be a Registered Nurse, hold a current driving licence and should have experience in looking after children and young people with complex health needs conditions, with a confidence in the use of central venous access devices.

Experience of working within the community is desirable.

The role will require, on occasion, forming part of a 24 hour on-call team for end-of-life care.
