Following investment, the Mental Health Liaison Service an exciting opportunity has arisen for a motivated and enthusiastic clinician to work within the dynamic Mental Health Liaison Team based at the Stonebow Unit, Hereford. The role will serve to identify and support patients who present within the acute hospital with a variety of mental health conditions alongside complex physical comorbidities. The clinical content of liaison psychiatry practice is complex, and every day brings a new challenge. We are seeking adaptable, flexible mental health practitioners who wish to develop their skills within the hospital setting (Newly Qualified Considered). The hours of work are 37.5 worked over 3/4 days but we may be willing to negotiate less hours to a minimum of 33 hours per week. Main duties of the job

It's a little unpredictable, as you can be called at any time to a crisis situation, we discuss referrals each morning and twice weekly we review the caseloads with our consultant psychiatrist. We complete comprehensive risk assessments for every patient, these can be completed independently although sometimes we discuss more complex patients as a Multi-disciplinary team. The team works hard to provide a fully integrated service for our patients, so it's not just about making sure their needs are catered for while they are in the hospital, but also when they leave. We talk to families and carers, GPs, community mental health teams, social care teams, community nurses, physiotherapists and voluntary organisations, and anyone who is necessary to ensure a full care plan is in place for a patient when they leave hospital, so the support they need at home can start the minute they arrive.

Job description Job responsibilities

• Operate as part of a multidisciplinary team providing Liaison Services to Acute Hospitals countywide for people aged 17 and over.
• Use their initiative and take responsibility for themselves and the quality of their work and the service they provide to patients.
• Proactively engage in formal and informal education of Acute Trust staff and other partnership agencies as well as appropriate students.
• Actively and creatively engage patients and carers encouraging them to make appropriate use of the mental health and voluntary services available as part of a process of recovery and crisis management.
• Undertake full psycho-social assessments with those in need of such. Ensuring thorough, precise and timely documentation and dissemination of information in line with Trust policy and procedure

Person Specification Experience Essential

• Undertaking mental state and self-harm assessments
• Assessing and managing risk
• Working within the Care Coordination framework

Desirable

• Experience of assessing people who deliberately self-harm and those diagnosed with personality disorder
• Experience of working with people who engage in substance misuse
• Experience of working with/assessing people with cognitive impairment and/or functional mental illness
• Experience of working in a Liaison or Crisis & Home Treatment service

Skills & Abilities Essential

• Ability to communicate clearly and effectively in English in both verbal and written formats.
• Computer literacy.

Qualifications Essential

• Registered Mental Health Nurse, Occupational Therapist or Social Worker
• A relevant degree or diploma
• Full Current UK Professional Registration

Desirable

• Appropriate additional post-registration/graduation qualifications
• Evidence of on-going continuing professional development
• RGN
