Gloucestershire Health & Care deliver community health services for adults & older people with multidisciplinary teams closely aligned with Primary Care Networks and the aspirations for place based integrated care. The Integrated Community Team consists of nursing, therapy and Reablement teams working closely alongside Adult Social Care colleagues to provide support to people within their own homes and within bed based Reablement units. We work closely with GPs, local communities, acute and community hospital(s) and other partners to enable people to live as independently and for as long as possible within their own home. We are a clinically led organisation where individuals are valued for their unique professional contribution to ensuring best patient care. Continuing Professional Development is a key priority and we actively promote and develop the broadening roles that Physiotherapists and AHPs are able to contribute.
As this is a community-based role you will be required to have a full Driving Licence valid for the UK and your own vehicle for business use.
Main duties of the job

You will need excellent communication skills and commitment to high quality patient care alongside flexibility and resilience to meet the challenges of community working. Opportunities exist for you to work with rapid access from our referral centre, bed-based reablement, 'Home First' and domiciliary based intervention, so there's plenty of scope for your professional development.

Our programme is provided to all Newly Qualified Nurses, Nursing Associates, Internationally Educated Practitioners, Allied Health Professionals and Return to Practice Practitioners. In addition to the programme we also offer all preceptees the opportunity of enrolling and completing the level 7 (Masters) 15 credit validated module Preceptorship for Healthcare (AP7026).
Newly qualified Social Workers are offered the Assessed and Supported Year in Employment programme (ASYE) which is similar to a preceptorship programme.
