Main duties of the job

You will bring:
  • A detailed knowledge of modelsand approaches used in supporting people to manage Long Term Health conditions
  • Significant experience of applying models using a wide range oftools andtechniques
  • A willingness to work as part of an interdisciplinaryteam
  • An ability to provide training and supervision to other professionals and physiotherapy students
  • Enthusiasm for research and development
  • Excellent digital skills including an ability to use the full range ofMicrosoft365 tools

Applicants will have extensive experience of working with and supporting people with Long Term Health conditions in order to help and empower them to develop self-management skills.

Candidates will be experienced in the use of technology comfortable working with people over the telephone or via video conferencing.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

  • Accredited post graduate specialist courses/ training relevant to role

  • Post graduate training
  • MSc (or working towards) in relevant specialty

  • Member of CSP
  • Relevant leadership courses/ qualifications
Experience Essential
  • Significant postgraduate experience
  • Significant experience of managing a complex caseload working within relevent speciality
  • Experience in multi-agency team working within specialist area
  • Leadership of relevant networks/ practice groups
  • Experience in audit and service evaluation
  • Experience in presenting to a range of audiences
