Be an active member of the Brent Falls Prevention Service.
To provide comprehensive multi-factorial falls assessment, treatment and evidence based programme of rehabilitation to all clients who have fallen as part of an inter-disciplinary service
To hold a clinical caseload delivering a client centred, evidence based occupational therapy interventions
Work within professional standards and clinical guidelines, promote best practice and to undertake all aspects of clinical duties as an autonomous practitioner including assessing and treating own caseload of patients/clients and maintaining professional documentation
Build up and maintain key relationships with referring services across CLCH including the other Falls and Bone Health Services in CLCH, Frailty Clinic at Northwick Park Hospital, Intermediate Care Teams, GPs, District Nurses, A& E, Ambulance Services and other community service providers to facilitate appropriate referrals and an understanding of the role of the Occupational Therapist in the Falls Intervention Service
Be responsible for the supervision of junior staff, Rehabilitation Assistants and Occupational Therapy students as the opportunities arise.
Work in a variety of settings including clinics, clients own home, community centres, extra care sheltered housing and continuing care homes

Main duties of the job
- To communicate relevant medical information, assessment details, advice and recommendations across agencies and with clients, to provide a coordinated service to meet the client's care needs, in compliance with Trust Guidelines for sharing of information and informed consent. These agencies include Acute Trusts, General Practitioners, Community Rehabilitation Teams, Social Services, other healthcare professionals, voluntary agencies, and care homes
- Demonstrate compassion, empathy and sensitivity towards clients who may have complex emotional and physical conditions such as fear of falling, anxiety, pain and other physical psychological and social concerns or when delivering unwelcome news around their condition or rehabilitation prognosis
- Comply with local risk assessments when dealing with complex clients with challenging behavioural needs and high risk extreme environments
- Be responsible for accepting allocated clients, organising and prioritising own caseload to meet service needs and to communicate with your supervisor if the service needs cannot be met.
- Prescribe appropriate equipment, following the Transforming Community Equipment Services procedures, to clients in order to maximise their independence

- To work collaboratively with all agencies in health, housing, Social Services, voluntary sector, and community leaders to implement sustainable evidence based practice and achieve the best care for all community clients
- To provide education and training on falls prevention and management to other community rehabilitation teams and service providers including GPS, District Nurses, Residential Care, Social Services and voluntary sector services.
- Provide clinical supervision, appraisal and competency attainment for the Rehabilitation Assistants in the team, by providing theoretical and practical clinical training, education sessions and advice on client needs and treatments
- Review available resources to update evidence based clinical knowledge for individual and service development. To advise and to educate referrers on these developments. To assist in other service promotion activities as delegated by the supervisor
- Be professionally and legally accountable for all aspects of own work ensuring that the standard of professional work meets professional standards and clinical guideline

- Diploma or degree in Occupational Therapy
- Current HPC Registration
- Post Graduate qualification in relevant field or equivalent combination of degree, short courses and experience
- Evidence of CPD relevant to the treatment of people with physical disabilities and those with falls risks factors.
- Up to date CPD Portfolio

- Training in appraisal and clinical supervision
- Member of Specialist Body e.g COT SS-Older people
- BAOT Membership
- PSI (Postural Stability Instructor) Accredited Training
Experience

- Experience of working in a falls team/ working with clients that have fallen, with a client group who are risk of falls or sound knowledge of falls.
- Experience of teaching and training to a wide range of people both formal and informal
- Post registration experience
- Experience in a wide range of clinical areas, e.g. rehabilitation with older people, in-patient and/or community rehabilitation, orthopaedics
Desirable
- Experience in a falls prevention team and intermediate care
- Experience of report writing and production of business reports
- Experience of supervising/appraising Junior therapists, unqualified staff or students
