An exciting opportunity has arisen in the Salford Mental Health Liaison Team (MHLT) for a Band 7 Senior Practitioner to join the team in a newly created role, leading on work with patients identified as being intensive users of the Emergency Department (ED) and Urgent Care services. The successful candidate will work as part of the MHLS leadership team and be an experienced, driven, and passionate individual, able to work collaboratively with both patients and wider agencies, to ensure that people are able to access the right care to meet their individual needs, thus reducing a reliance on ED attendances. The successful candidate will have a proven track record demonstrating comprehensive assessment, risk formulation and care planning, working with individual's presenting with complex needs.

Main duties of the job
- Provide clinical leadership, supervision, support, and consultation to nursing staff and MDT staff in the MHLS team, in line with relevant clinical pathways as outlined within the operational procedures.
- Support patients who are high intensity users of urgent care services through developing excellent links with the acute hospital Trust and wider agencies, ensuring a strong multi-agency approach to care planning that recognises the often complex physical and mental health needs of this patient group.
- Take a lead clinical role in the completion of highly developed assessments, risk management, interventions and onward care planning of patients identified as intensive users of the ED.
- Recognise the importance of individualised care planning and, wherever possible, work collaboratively with patients, families & carers to identify and plan ongoing care needs.
- Develop robust systems to effectively monitor and evaluate the impact of work completed, using both qualitative and quantitative feedback.
- Work as part of the wider MHLS, offering biopsychosocial assessment, risk management, brief intervention and care/discharge planning, to a wide range of patient groups, across the hospital footprint.

Education/Qualifications
Essential
- Registered professional with relevant professional registration (NMC, SWE, HCPC)
Desirable
- Non-Medical Prescribing or willing to complete this training
Experience
Essential
- Have a minimum of 3-years post qualifying experience
- Have experience of working within liaison services/assessment within A& E Department /General Hospital.
- Have experience in the management of complex cases, relating to physical and mental health
- Have experience of participating in audit and an understanding of implementing Action plans relating to the outcome of audits.
- Have a varied range of experience which may include dementia care, acute care, supporting people who self-harm and misuse substances.
Desirable
- Have experience / training in Psychosocial interventions.
Knowledge
Essential
- Have knowledge of the background, range of services and evidence base of Liaison Psychiatry
- Have a sound knowledge of the social factors involved in the development of ill-health and key features of relapse prevention
- Have a working knowledge of the recovery and rehabilitation models and mental health promotion.
- Be able to show an understanding of current national policy and initiatives in relation to severe and enduring mental health issues, dementia, self-harm and substance misuse.
Skills & Abilities
Essential
- Be competent in the area of risk assessment and formulation of risk management plans including supporting patients who self-harm or are in a neglectful state
- Be competent in the application of Bio/psychosocial assessments across a varied patient illness profile including dementia, functional illnesses, self-harm and substance misuse
- Be able to undertake a complex formulation and discharge/care plans for a wide range of people including those with dementia, functional illnesses, self-harm and substance misuse
- Possess excellent verbal and written communication skills with a wide range of people including professionals, patients and carers
- Effective and proven Leadership skills.
