Job Summary2.1 To improve the lives of people with learning disabilities by minimising the impact oftheir condition through the delivery of excellent services to promote recovery and well being.
Main duties of the job
An exciting opportunity to join a highly experienced multi disciplinary team within the Durham locality working and supporting people with Learning Disabilities to achieve high quality of life and optimum health .The role is based within Seaham, spectrum business centre and currently offers flexibility to work from home following up to date policies and guidelines.
The successful candidate will carry a case load and act a Care Coordinator /Community Nurse and participate in the out of hours nurse on call rota .
This is a perfect opportunity to gain skills and specialist knowledge across the spectrum of health and social care.
The post is open to newly qualified staff and we support preceptorship .
