Want to develop your leadership skills, with the support of a Band 7, to support an enthusiastic and developing team?

If you are passionate about providing high quality nursing care to people in their own homes and want to support individuals to remain in their preferred place of care, preventing unplanned admissions, then the Out of Hours Nursing Team within Community Health and Social Care is for you.

The Team supports the community populations across Exeter, East and Mid Devon the Team are based in Ottery St Mary.

We are seeking to recruit enthusiastic, experienced nurses to the Band 6 role of Community Nurse Specialist. You will be able to demonstrate your enthusiasm and commitment to community nursing, highly developed clinical skills to share as the clinical expert and be able to evidence your leadership skills/experience to support junior team members. The post-holder will be supported by the Team Manager and a Clinical Matron.

Our Out of Hours Team, consists of nurses and healthcare assistants. The team provides unplanned care to ensure we support the needs of our populations around the clock, undertaking responsive holistic nursing and social care as required. The caseload covers a broad range including end of life care, admission avoidance and support for timely hospital discharge. The team also provide clinical support for our Urgent Community Response support workers between the hours of 5pm 10pm.

This post provides a huge opportunity to harness the enthusiasm of the current team to develop the service further, enhancing the nursing skills of junior staff to broaden the provision available to patients. Support will be given to undertake the SPQ qualification.

Applicants will be required to commit to all shift patterns within the team, however preferences will be taken into consideration applicants will need to be comfortable with driving, sometimes long distances across the geography the team uses Trust pool cars.

Working Pattern: The team currently provides a service from 16:00 08:00 hours worked over 2 shift patterns 16:00 00:00 hours and 22:30 08:00 hours consideration will be given for applicants expressing a wish to work 12 hour shifts.

Pay - The salary advertised for this position reflects the Agenda for Change 2024-5 rates, inclusive of a 5.5% pay award implemented from October 2024. If you join before this date, the pay award will be backdated to April 2024 or your start date, whichever is later. Until then, remuneration will be based on the 2023-4 pay bandings (Pay scales for 2023/24 | NHS Employers).
