Glemsford Surgery are recruiting to Practice Nurse Role. It is a surgery underneath the Agenda for Change process and part of West Suffolk NHS Foundation Trust.
As a Practice Nurse you will be responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with long term condition and the provision of preventative healthcare to the practice population.
Main duties of the job
You will work within the general practice team to meet the needs of patients and support the delivery of the services. You will be expected to demonstrate clinical and communication skills to a high standard and a commitment to improving nursing practice through on-going personal and professional development.
It is preferable for the post holder to be able to do cervical cytology and respiratory issues (asthma reviews, COPD reviews and spirometry performance and interpretation). Also childhood immunisations, vaccinations, travel vaccinations, wound care including doppler, compression bandaging and smoking cessation.
Experience of SystmOne system is desirable. The candidate will need to have a flexible working approach and have the ability to work with a wide range of professionals, people and patients.

JOB PURPOSE:
The practice nurse will provide evidence-based nursing services to the practice population in accordance with agreed national / local guidelines and the NMC Code of conduct.
They will work as part of the primary care team and be able to work both independently and collaboratively with other professionals to provide quality evidence-based care.
KEY DUTIES:Professional responsibilities / Development
To work in accordance with current local and national protocols and guidelines.
Maintain and develop knowledge by participating in educational opportunities including online training, peer to peer forums and attending educational events relevant and necessary to professional development.
To have the ability to work on own initiative, acknowledging limitations and seeking advice or referring to other colleagues or health care professionals where needed.
To keep up to date with the changes within primary care nursing and be able to evaluate and adapt care using evidence-based practice.
Participate in an annual individual performance review and maintain a record of professional development in accordance with the NMC revalidation process.

Clinical Duties
To include, but not limited to:-
oAdministering prescribed injections.
oChildhood and Adult Immunisations in accordance with Patient Group / Specific Directions (PGDs / PSDs), or as prescribed.
oPerforming ECGs.
oMonitoring Blood Pressure, Blood Glucose and Cholesterol.
oCollection of specimens - phlebotomy, urinalysis, swabs.
oCervical cytology, in accordance with national screening programme.
oContraception reviews of oral, patch, ring and injectable devices.
oAssessment and syringing of ears.
oPerforming a Travel Heath risk assessment, administering recommended immunisation, arranging antimalarial prophylaxis and providing general health care advice and information.
oWound management.
oMinor injuries management.
oAssisting with minor procedures - operations / contraceptive devices.
oCarry out diagnostic Spirometry and interpretation of results.
oPerform Doppler studies and interpret results.
oHealth checks / assessment - New Patient, NHS, Severe Mental Illness, Learning Disabilities, Memory tests (GP COG).
oHealth Education - smoking, alcohol, diet, exercise, weight management.
oHealth Promotion - promoting healthy lifestyles, well person checks, CVD risk assessment (Q risk) and encourage patient participation in national screening including Breast, Cervical, Bowel.
oLong Term Condition reviews (to appropriate level) - Diabetes, Cardio / Cerebral Vascular Disease, Peripheral Vascular Disease, Heart Failure, Hypertension, Asthma / COPD.
To provide telephone advice to patients / parents / guardians / relatives and occasional home visits where appropriate and within levels of competence.
Any other duties as reasonably requested by the doctors / nurse manager providing these duties are within the scope of the role and that the individual has undertaken the appropriate training and is competent to undertake the duty.
Professional Duties
To keep accurate and comprehensive computerised medical records using appropriate read codes / templates.
Ensure all documents relating to patient care are completed accurately i.e. immunisation records / certificates.
To access and file microbiology / cytology results using appropriate clinical information sources i.e. Open Exeter / ICE.
To work cooperatively in the provision of care to meet Quality Outcome Frameworks (QOF) and Enhanced Services (DES / LES).
Refer patients to other services including Mental Health / Wellbeing, Drug / Alcohol, Weight / Exercise management, Smoking cessation, Podiatry, Physiotherapy, Occupational Therapy, District Nursing, Continence Service, Health Visiting and Leg ulcer / Chronic wound, COPD and Heart Failure management clinic.
Understand the importance of effective communication within the team and strive to communicate effectively with other members of the practice, patients and their parents / guardians and carers.
Ensure infection control procedures are followed and protocols are adhered to at all times. Any risk should be actioned and / or reported as indicated to the appropriate person.
To be familiar with and follow protocol for the safe and effective running of the practice concerning all aspects of health, safety and security including data protection and fire safety and to undertake any recommended training.
In participation with the nursing team ensure the maintenance of equipment, stock levels and of the safe running of the treatment room.
Act accordingly to achieve the cold chain and maintain stock levels of vaccinations. In participation with the nursing team order vaccinations required and record daily fridge temperatures.
Attend annual resuscitation training, be familiar with emergency equipment and drugs in the practice, and in participation with the nursing team maintain / check equipment and ensure drugs are up to date and equipment is fully functioning.
Support, coordinate and participate in health promotion campaigns i.e. Flu. Participation in annual influenza campaigns or other Public Health campaigns may include paid overtime at evenings and or weekends.
Participate in practice meetings, audit and research projects.
Assist with the collection of statistical information / data collection required to provide quality care.
Participate in the education of students and the induction of staff of all disciplines (where appropriate).
