End of Life Care within Walsall is rated as OUTSTANDING by CQC within community and the Hospice. The post holder will work within a multidisciplinary team who are focused on delivering the best holistic care possible to patients with a life limiting illness.Goscote Hospice located at Goscote Lane, Palliative Care Centre is integrated with the Palliative & End of Life Care services provided by the Trust, which include community nursing, day hospice and hospital and community palliative care nursing , Consultants, Allied Health Professionals. Main duties of the job

37.5 hours per week.

• Caring for patients with a life limiting illness likely to be in the last 12 months of life
• Support symptom control management for patients attending with physical, psychological , emotional symptoms
• Care for patients in the last days/weeks of life
• Care for patients admitted for symptom management
• Be part of wider Specialist Palliative Care MDT
• Deliver fundamentals of care based on patient wishes
• Support communication with carers
• Link with teams across acute , community & primary care
• Clinically lead the team on a day to day capacity and in the absence of the Band 7 lead nurse
• Complete quality audits

Undertakes the provision of nursing care with support from senior colleagues. Will develop the skills and competencies to perform additional skills as related to the specialism. Will assist in the supervision of junior nursing staff and, where applicable, the teaching of students from differing disciplines eg nurses, AHPs paramedics.
