A rare and exciting opportunity has arisen to join Herefordshire Community Learning Disability Teams as a Band 6, 36 hours per week. Part-time will be considered. The team is based at Liverpool Court, Commercial Road, Hereford.

The successful applicant will provide assessment, diagnosis and develop bespoke therapy for a caseload of service users with a learning disability with complex needs, using evidence based/ client centred principles and delivered in accordance with HCPC and College of Occupational Therapy (COT) requirements.

You will be joining a small but tenacious Multi-Disciplinary team who are all committed to ensuring that the physical and mental health, communication and occupational needs of individuals with a learning disability are equitably met.

The Occupational Therapy Team across Herefordshire and Worcestershire are innovative in their service delivery using a mix of face-to-face visits, remote working, and social media to support services users. We meet regularly to offer peer support and to look at clinical/service issues, developing resources and regular CPD events, training sessions and face to face away days.

You will receive regular individual and group supervision, training, and support within this role to help transfer your skills and develop competencies in this specialist field.

Main duties of the job

Manage a caseload of service users with complex needs, using evidence based/ client centred principles.

Assess, plan, implement and evaluate interventions.

Participate in the planning, development and evaluation of Occupational Therapy Services. Provide outcomes for the service and service users by using agreed tools/assessments.

Contribute to the development and maintenance of the Trust Occupational Therapy Service. Ensure OT provision is delivered in accordance with HCPC and College of Occupational Therapy (COT) requirements.

Job description

Job responsibilities

Hold a caseload this will include those people with complex Occupational Therapy needs.

For people on their caseload, to be responsible for the whole process of assessment, care planning, monitoring and review.

Act as an experienced member of the CLDT in relation to risk assessment giving advice and direction as required.

Contribute to Continuing Healthcare Assessments.

Monitor, evaluate and modify interventions in order to measure progress and ensure effectiveness.

Provide education, consultation and advice to care settings and day activity placements.

Have knowledge of legislation pertaining to Learning Disabilities eg mental capacity Act, Best Interest, Valuing People, death by indifference.

Carry out complex risk assessment involving wide consultation and collaboration.

Able to carry out a moderate physical effort throughout the working day.

Be required to work with people who have needs around challenging behaviour

Be required to carry out complex assessments and interventions in difficult circumstances, e.g., poor environmental conditions, or at times when service users' behaviour is unpredictable.

Be required to deliver complex and sensitive information/messages to service users, carers and families.

Be required to travel independently in and around the community

• Qualification approved by the Sensory Integration Network in Sensory Processing Disorder
