** Due to internal promotions and service expansions, we have vacancies for Senior Nurses/Paramedics within the West Hertfordshire UCR service **

We are looking for forward thinking, passionate staff who can take a positive risk management approach to support people at risk of a hospital admission to remain at home when acutely unwell, within our UCR service in South & West Hertfordshire.

The aim of the service is to avoid unnecessary admissions into secondary care and provide a timely discharge from hospital and community bedded units for patients requiring urgent input in their usual home environment. The team provides care across three pathways delivered over 7-days a week 8am to 10pm - Rapid Response (RR), Early Intervention Vehicle (EIV) and Discharge to Assess (DTA). Unplanned Care also include the Frailty Hospital at Home pathway.

Main duties of the job

Work on a rotational shift basis within the Herts Unplanned Care Team - service operating hours 8am 10pm 7 day a week.

Be the clinical leader for designated shifts on a rotational basis responsible for day-to-day management of the service.

Take designated responsibility for discrete areas of operational management, line management and clinical practice.

Prioritise and manage own workload with minimal supervision.

Education/Qualifications

Essential

  • Registered Nurse / Paramedic with current NMC / HCPC registration.


Desirable

  • Evidence of continuing professional education and training at Masters Level.
  • Further desirable qualifications in Primary Care/ Accident & Emergency/ acute clinical assessment.
  • Qualification in teaching or practice-based learning.


Experience

Essential

  • Extensive experience in Primary Care and / or Accident & Emergency.
  • Proven experience of utilising expert diagnostic and clinical skills to provide autonomous, unscheduled medical care and management of patients presenting with undifferentiated and undiagnosed conditions.
  • Experience of assessing and providing quality care to people with Long Term Conditions.
  • Track record of practice excellence.
  • Effectively working in collaboration, negotiating, and liaising across organisational boundaries.
  • Experience in monitoring clinical standards.
  • Proven ability to prioritise and manage own workload with minimal supervision.
  • Determining best ongoing care for patient.


Desirable

  • Team / service/ staff management.
  • Experience of implementing evidence- based care and developing others to do so.
  • Conducting research projects and audit.
  • Change management and implementing change in practice.
  • Evidence of innovation in practice.


Skills & Knowledge

Essential

  • Evidence of comprehensive range of clinical skills and expertise relevant to the role.
  • Competent in physical examination of: ENT, eye, gastrointestinal, respiratory, cardiovascular, neurological, skin, musculoskeletal systems and wound management.
  • Confident in assessing acute MSK presentations.
  • Specialist knowledge in clinical reasoning, history taking, physical examination, diagnostic skills and prescribing/treatment.
  • Basic IT skills including use of Microsoft Office package, and email.
  • In depth knowledge of pharmacology.

Essential

  • Car driver. Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
