We are recruiting for a Lead Nurse (Band 6) to join our MPFT - Diabetic 8 Care Team. (Band 5 Nurse may also be considered).Please note this is a full time post but 30 hours p/w may also be considered.If successful, you will be managing a team and supporting the delivery and oversight of the 8 care process checks for people with diabetes and ensuring a high quality service for people across Staffordshire and Stoke on Trent.

This role will work within the Diabetic Eye Screening Programme service to oversee the provision of the 8 care process checks to those patients across Staffordshire and Stoke on Trent who are at most in need and are at higher health risk of complications associated with diabetes.

Due to the urgent nature of the recruitment into this post, we would not be able to accept overseas applications at this time.

Main duties of the job

o The post holder will be part of the Diabetic 8 Care Process Team, providing nursing care and will work in a variety of settings.

o This is a dynamic post that requires the post holder to work closely with uni/multi-disciplinary teams/agencies in providing health care screening, preventing unnecessary hospital admissions and improving the health and wellbeing of the population served.

o Responsibility for team co-ordination will be assumed in the absence of the Operational Lead/Manager.

o The post holder will be skilled in the assessment of care needs ensuring that the patient is able to access ongoing treatment for needs identified.

o They will lead in organising the induction of clinical supervision of students and new team members and act as a role model to the team.o This role expects the nurse to clinically lead the 8 Care team and the post holder will be in charge of that caseload with virtual and actual support from the Operational Lead under supervision.

To assess new service user referrals and care & support needs.Having oversight of and undertaking delivery in relation to the 8 care process checks. The checks that will be expected to undertake are as follow:

Blood Pressure Cholesterol Serum Creatinine Urine albumin Foot surveillance BMI patient height and weight Smoking HbA1c

To use expert knowledge and judgement to take into account the range of care/support packages and treatment options available. To consider each service users personal circumstances and preferences in order to implement and adjust programmes of care and intervention.

To maintain and develop skills and knowledge to deliver a range of interventions and sets an example of good practice to other team members including skill sharing.

Must be capable of cost and time efficient travel across a dispersed service area, this post will involve working across MPFT Staffordshire. A car user with full driving licence would be required due to areas covered in this role. Please see attached Job Description & Person Specification.

  • Degree in nursing or equivalent experience.

  • Evidence of further clinical study.

  • Relevant experience to the role including significant experience working in a community setting.
