This post offers an exciting opportunity to work within an innovative model of nursing care - the PATCH team Providing Assessment and Treatment for Children at Home.
Aims of the Role
Provide nurse led care in the form of telephone reviews and/or home visits for further assessment to children who would have otherwise been admitted to hospital.
Deliver health education and promotion to children and their families at home during an acute episode of illness to reduce PED re-attendances and help build resourcefulness within the community.
Continue to nurture existing working links between the Paediatric ED ( PED), Paediatric assessment Unit (PAU)ED, and community teams to ensure seamless pathways for children mildly-moderately unwell with common conditions.
Audit and review existing pathways of care for common minor conditions and deliver and disseminate associated educational resources to professionals working across the urgent care pathway for children
Main duties of the job
Use and develop his/her clinical experience and skills in the independent assessment and management of acutely unwell children to ensure all children referred to PATCH team receive high quality, effective, individualised care that is compassionate and caring.
Have opportunities to work within the Paediatric A& E Dept, Paediatric Assessment Unit and the children inpatient ward at The Hillingdon Hospital NHS Trust and with the local Children's Community Nursing Teams to develop and refine their skills and knowledge.
Assist in the reduction of acute admissions to inpatient hospital beds by supporting children and families in the community to self-manage conditions and by providing hospital at home care therefore has a full driving license and willingness to travel to visits independently.
Work closely with the lead nurse/practitioner, consultant and multi-disciplinary team members both in the hospital and community to monitor and maintain policies, standards of nursing care and staff development.
Be flexible in the approach to the clinical and community area and provide support for colleagues, junior staff and students.
Assist in the reduction of acute admissions to inpatient hospital beds by supporting children and families in the community to self-manage conditions and by providing hospital at home care.
