An exciting opportunity has arisen in the Holistic Healthcare for the Homeless Team (HHH) for the position of a Non-Medical Prescriber, Mental Health.
The Team is open to all individuals that are rough sleeping or who are vulnerably accommodated within the City of Lincoln. The team provide both physical and mental health care to the rough sleeping community and those vulnerably housed, as this cohort of people have difficulties in accessing mainstream services and are the most disadvantaged of our population. The service is an enhanced service to existing GP provision.
Main duties of the job
To proactively participate in the development of an efficient and effective Holistic Health for the Homeless Service.
To ensure the health needs of people who are rough sleeping (both acute and chronic) are met in order to promote wellness and prevent ill health, and early death.
To use advanced nursing skills and knowledge to holistically assess patients' needs and to provide clinical treatments based on evidence based practice.
The role involves providing advanced nursing care to the rough sleeping population both within a clinic setting and when on Street Outreach.
To proactively engage with hard to reach groups through Street Outreach Teams , Hostel staff and wider Neighbourhood Team members

To use advanced nursing skills and knowledge to carry out physical and psycho-social assessments for the rough sleeping population
To provide advanced clinical / therapeutic treatments based on best possible evidence that will improve health outcomes for the rough sleeping population.
To undertake non-medical prescribing and to ensure that other duties in relation to medicines management meet required professional standards
To work in partnership with other community teams/voluntary organisations in order to ensure the seamless delivery of care to the rough sleeping population and Neighbourhood working to support transition
To act as an advocate for people who are rough sleeping, supporting patients own choices in regards to their care whilst promoting independence and self-care
To constantly champion the principles of self-care and patient empowerment.
Act as a mentor/supervisor as required for non-medical prescribers within the service
Attendance at leadership and service steering groups
To take the lead in the delivery of specified educational programmes, on topics such as patient assessment, acute nursing care
To proactively identify service users who would benefit from the rough sleeping health service
To be in regularly communication with all members of MDT team
To proactively develop and maintain close working relationships with the Wider Neighbourhood working including all relevant stakeholders
Provide training in dual diagnosis and mental health treatment to other professionals

Qualifications
Essential
- 1st Level Registration (NMC,)
- Non medical prescribing qualification (V300)
- Master level qualification in advanced clinical practice, or working towards this qualification
Experience
Essential
- Experience, working in primary or urgent care and also experience working as an Advanced Nurse Practitioner.
- Advanced theoretical and practical knowledge of managing a wide range of clinical conditions gained as a result of additional training and work experience in primary care
- Experience of prioritising complex workloads.
- Experience of applying relevant legislation and national guidance within nursing practice
- Experience of working collaboratively in a multi-disciplinary team
Desirable
- Experience of assessing and prescribing treatments for mental health problems

- Must demonstrate a working knowledge of health, education, social services and the voluntary sector and have the ability to work in partnership with other professionals from a range of multi-disciplinary backgrounds
- Must demonstrate knowledge of current legislation including the Human Rights Act 1998' safeguarding of children and adults, Mental Health law and policy and the Mental Capacity Act 2005.
- Demonstrate the application of values to mental health practice including the ability to identify, challenge and where possible redress discrimination and inequality.
- Must demonstrate knowledge of safety factors in the management of self-harming behaviours and safeguarding issues and have the skills to be able to apply knowledge appropriately with complex cases.
- Ability to deal with complex issues both systematically and creatively, making sound judgments and decisions in the absence of complete data and communicate their conclusions clearly to specialist and non-specialist audiences
- Demonstrate knowledge of a range of models of behavioural and mental disorder including the contribution of social, physical and development factors, the social perspective on mental disorder, the and the implications of mental disorder and treatments for clients, their relatives and carers
