An exciting opportunity has arisen within our Urgent Community Response/Virtual ward Service for a Senior Nurse, 32 hrs per week, 12 month fixed term contract.

Our UCR/VW is an evolving service delivering acute assessment, management and monitoring within the community setting and we are looking for a dynamic forward thinking individual to join the team.

The post holder will work collaboratively within the multi disciplinary team to offer holistic high level clinical assessments and reviews for patients who may be in a crisis situation.

Main duties of the job

Urgent Community Response is a service operating a multidisciplinary approach, supporting patients in their own home for 72 hrs and so preventing ED attendance and unnecessary hospital admissions. The team is made up of Senior Therapists, Senior Nurses, Social Workers, Nurse Associate, Community Pharmacist and Senior Support Workers.

o The role of the Senior Nurse is to work collaboratively with other team members offering holistic high level clinical assessments and reviews for patients who may be in a crisis situation. There is a strong element of See and Treat management within the nursing role in the community setting. of care to prevent hospital admission

o Demonstrate a high level of judgement, discretion and decision making within the assessment process with an aim to reduce hospital attendance/admission

o Develop and deliver educational/training programmes and act as a specialist resource for staff and colleagues to improve standards and quality of care.

o Monitor and improve standards of care through supervision of practice, clinical audit, evidence based practice, learning and development and the provision of skilled clinical leadership

o Support the service lead with service improvement development

Job responsibilities

The post holder has continuing daily responsibility and accountability with supervision of the support staff within Urgent Community Response Team (UCR).

Assess patients within a Crisis situation and offer appropriate nursing assessment intervention, and review to meet their needs using advanced clinical assessment skills incorporating non medical prescribing.

To offer a competence within a see and treat model that would enable the patient to remain well at home and avoid admission to hospital.

To assess the environment/social situation and identify any risk that could potentially lead to hospital admission.

Assesses, plan, implement and reviews routine through to complex nursing care plans using research and evidence-based knowledge to underpin practice and which support avoiding a hospital admission.

To supervise, lead, inform and delegate staff as appropriate to the needs of the service by utilising skills and resources effectively as directed by the Urgent Community Response team leader.

To be an expert resource within the team regarding all aspects of nursing care.

Applies Specialist, analytical and judgemental skills to assess and interpret acute, chronic and palliative conditions, taking appropriate actions to prioritise patients needs.

Demonstrates a high level of intuitive and creative problem-solving skills to facilitate patient centred care rationalising information to create patient concordance by using a high level of communication/negotiation skills to affect the outcome.

• Provide support for the team across working hours to manage expected and unexpected clinical events within the scope of practice.
• Supervise and monitor staff rostering and skill mix to maximize effective delivery of care.
• Ensures risk assessments are completed and actions implemented to meet the health and welfare needs of staff and patients.

• Ensure national and local targets are reported through the organisations reporting mechanisms.

• To regularly monitor and report on quality assurance within organisational frameworks.

• Assess, plan and implement packages of clinical care to patients/clients within own sphere of practice in a variety of community settings.

Education & Qualification

Essential

• 1st Level Registered Nurse
• Degree qualification or experience to demonstrate level of working
• Urgent care / emergency room experience or educational qualification
• Prescribing Qualification NMP V300 or ability to work towards
• Advanced Clinical Assessment Skills qualification at Level 7 or experience to demonstrate level of working

Desirable

• Teaching/mentorship qualification
• Community Specialist Practitioner or equivalent qualification

Knowledge

Essential

• Knowledge of Government legislation and guidance pertaining to the role
