Cornwall Foundation NHS Trust looking to increase the number of Clinical Leads already working in the Initial Response, Home Treatment Team and Crisis Hub based in Redruth.

The Clinical Leads will provide expert advice, line management, clinical leadership, caseload management, supervision and training to the team and will be responsible for ensuring the consistent provision of high quality mental health care to people in mental health crisis in the community.The Clinical Leads will provide expert clinical knowledge and prompt bio psychosocial assessment and treatment for the Crisis Hubs, HTT and Initial Response service patients with mental health or self- harm problems.

The Clinical Leads will provide direct patient care to people in their home/ Crisis Hub who are acutely mentally ill and who do not require immediate physical health care. The post holder will be responsible for the rapid response to referrals, face to face assessment and gate keeping of admissions to hospital and facilitating discharge from in-patient services once the risk to the individual or others has decreased. The post holder will also be expected to undertake urgent front line assessments from GP's and other referrers such as police, ambulance and local authority and make clinical decisions around appropriate care pathways.

This is a full time post, working 13 long days/nights over a 4 week period.

Main duties of the job

The Clinical Leads will provide expert advice, line management, clinical leadership, caseload management, supervision and training to the team and will be responsible for ensuring the consistent provision of high quality mental health care to people in mental health crisis in the community. Work closely with the Team Manager to ensure the effective management and development of the service.Provide expert clinical knowledge and prompt bio psychosocial assessment and treatment for the Crisis Hubs, HTT and SPOA patients with mental health or self- harm problems.Provide direct patient care to people in their home/ Crisis Hub who are acutely mentally ill and who do not require immediate physical health care. Responsible for the rapid response to referrals, face to face assessment and gate keeping of admissions to hospital and facilitating discharge from in-patient services once the risk to the individual or others has decreased.Undertake urgent front line assessments from GP's and otherreferrers such as police, ambulance and local authority and make clinical decisions around appropriate care pathways.Demonstrate excellent risk assessment skills whilst also applying a holisticapproach to assessing the health and social needs of people experiencing severe and enduring mental illness.

Person Specification Qualifications /Experience Essential
  • Professional qualification - RMN /OT / SW
  • Non Medical Prescriber or willing to work towards
  • Extension experience of conducting clinical assessments including risk assessments
Skills & Aptitude Essential
  • Comprehensive Risk Assessment Skills
Knowledge and abilities Essential
  • Demonstrate knowledge and understanding of the principles and knowledge base underpinning crisis assessment and intervention
  • Sound knowledge of the Mental Health Act 1983 and other mental health legislation
