CDDFT Community Respiratory Nursing Service is pleased to advertise a rare, 1 year, exciting secondment opportunity to join the Community Respiratory Specialist Nursing service as a staff nurse working across North Durham and develop skills in respiratory nursing including home oxygen review.

The successful postholder will deliver respiratory care in out-patient clinics as well as in their own homes, supporting the specialist respiratory nursing service.

The successful candidate will utilise their outstanding organisation and communication skills to achieve best patient outcomes whilst being well supported by expert respiratory specialists to develop a range of competencies in delivering excellent respiratory care and support for this group of complex patients.

The sucessful candidate will receive mentorship and opportunities to develop their learning and skill in the specialist field.

This post offers weekday working and flexible hours can be negotiated.

Main duties of the job

To support the respiratory specialist nursing team to manage complex respiratory patients in the community.

Support community teams with complex respiratory patients.

Take responsibility for the assessment of the health needs of patients under community respiratory care and manage a caseload of respiratory patients.

Offer chronic disease management of respiratory patients including COPD exacerbation pathway patients and virtual ward.

Perform Diagnostic Spirometry.

Perform oxygen review assessments.

Signpost patients and colleagues to appropriate services and escalate as needed.

Provide specialist advice to patients/carers.

Maintain records.

Accepts the responsibility for decisions made and actions taken.

Skilled at assessing and managing risk.

No requirement to manage/monitor budget.

MAIN DUTIES AND RESPONSIBILITIES

Management and Leadership

Clinical Responsibilities

To co-manage a case load of complex respiratory patients, under direction of the community respiratory nursing team, across the community respiratory specialist service, within clinic and patients home environments, Monday-Friday 9am-5pm.

To undertake holistic patient respiratory assessment and review including social and psychological well-being and progress treatment interventions and adapt interventions, within set protocols, to progress treatment interventions and to meet the changing needs of the patient.

Develop, implement and evaluate patient treatment plans.

Communicate patient treatment plan changes to respiratory specialist nursing team.

Understand basic respiratory anatomy and pathophysiology and the function and management of all respiratory conditions including mental well-being.

Diagnostic spirometry performance qualification and registration either through The Association for Respiratory Technology and Physiology (ARTP) or at certified ARTP standard, is desirable but not essential as training will be provided.

Perform capillary blood gas testing and oxygen reviews.

Provide long term condition care following the COPD pathway.

Accurately record clinical data and understand its significance including heart rate and rhythm, respiratory rate, pulse oximetry, peak flow rate, and, blood pressure.

Complete effective record keeping which demonstrates the decision making process, care planned and given, as well as patient outcomes in accordance with trust policy and legal requirements.

Understand the benefits of non-pharmacological interventions for respiratory illness/conditions including smoking cessation, weight management and nutrition, physical activity and brief cognitive behavioral therapy (CBT).

Understand motivational skills and empathetic communication.

Promote health and healthy informed choices.

Review smoking status, provide very brief advice and refer to appropriate smoking service.

Have knowledge of the benefits of exercise and pulmonary rehabilitation and refer to appropriate local rehabilitation programs.

Review and train inhaler technique to patients, carers and other health care professionals.

Participate in service improvement and development initiatives.

Have awareness, knowledge and implementation of safeguarding processes in relation to children, vulnerable patients and carers.

Developing service user/carer education materials.

Experience

Essential

  • Up to date knowledge of professional practice.
  • Knowledge of national and regional policies influencing respiratory disease.

Desirable

  • Post registration experience of working with respiratory patients.
  • Experience of working in a community setting.
  • Experience of audit and/or service development/review.


Statutory Registration

Essential

  • Nursing Midwifery Council part 1 and 2 Registration.

Qualifications

Essential

  • Registered Nurse.
  • Professional knowledge acquired through degree or equivalent level of demonstrable professional knowledge/experience.


Desirable

  • Cognitive Behavioural Therapy qualification.
  • Accredited respiratory course.
  • Diagnostic spirometry performance qualification and registration either through The Association for Respiratory Technology and Physiology (ARTP) or at certified ARTP standard.
  • Knowledge of supplementary oxygen assessment and blood gas performance.
