An exciting opportunity has arisen for an experienced and motivated Digital Change Manager with proven experience in leading cultural change and influencing staff at all levels to join our newly established Digital Patient Pathway Management team. This innovative team is dedicated to supporting, enabling, and optimising digital patient pathway management across the Trust.
As a Digital Change Manager, you will play a pivotal role in managing the people side of change, ensuring that digital initiatives are effectively adopted and embedded across the organisation. Working closely with clinical and operational teams, you will be responsible for leading the cultural and behavioural shifts needed to successfully deliver digital transformation projects.

As a Digital Change Manager, you will play a pivotal role in managing the people side of change, ensuring that digital initiatives are effectively adopted and embedded across the organisation. Working closely with clinical and operational teams, you will be responsible for leading the cultural and behavioural shifts needed to successfully deliver digital transformation projects.
Main duties of the job
- Applying your detailed knowledge and experience of previous NHS involvement to develop and implement strategies for change, transformation, training, communication and benefits realisation management, contributing to successful Digital Patient Pathway Management implementation ensuring alignment with the Trust's wider strategic goals.
- Build strong relationships with clinicians, administrators, IT teams, and patients, ensuring clear and consistent communication about the vision and benefits of digital change.
- Lead initiatives to foster a culture of digital innovation, ensuring that staff at all levels are engaged, supported, and empowered to adopt new technologies and processes.
- Design and deliver targeted training and support programs to ensure staff are equipped with the necessary skills and knowledge to successfully implement digital tools within patient pathways.
- Conduct assessments to understand the impact of digital changes on staff, patients, and workflows, developing tailored mitigation strategies to minimise disruption.
- Track the effectiveness of change initiatives, providing regular reports and insights to senior leadership on progress, challenges, and outcomes.
- Identify potential risks related to organisational change and develop mitigation strategies to ensure the smooth adoption of digital solutions.
