An exciting opportunity has arisen for a Band 6 Registered Mental Health Nurse to join the re-established Assertive Outreach Team within the Scarborough Community Team. You will benefit from a reduced caseload due to the complexity of the patients you will support and will work in collaboration with other professions within the team to form a holistic approach to patients care, treatment and recovery.
The applicant will provide evidenced based care and treatment within a recovery framework for adults with mental health conditions who find it difficult to engage due to their illness or other external factors such as housing or substance abuse.
You will need to be enthusiastic and highly motivated with experience in assessments and delivery of care for patients in a community setting. You will have access to clinical supervision within the team and will work closely with other professionals in the multi-disciplinary team each day.
There are varied opportunities for training and development, and peer support and supervision systems are in place.
Main duties of the job
To improve the lives of people with mental ill health by minimising the impact of their condition through the delivery of excellent services to promote recovery and well being.

To promote social inclusion, community access and participation through the provision of patient care.
To facilitate others in the team to develop competence by providing leadership, day to day supervision, clinical advice and clinical supervision to associate practitioners, community support workers and students as appropriate.
To be committed to and involved in activities integral to the Trust's QualityImprovement System.
To promote at all times a positive image of people with mental ill health.
To promote at all times a positive image of the Adult Mental Health Service and the wider Trust.

- Evidence of Continuing Professional Development.
