Band 5 - Children's Community Nurse - fixed term 10 months (to cover maternity leave)
The Children's Community Nursing Team is integral to the Ambulatory Model that is offered and delivered within the Tameside and Glossop locality. The team accepts direct referrals from the emergency department, children's observation and assessment unit, children's ward and local GP's working hard to facilitate early discharge from hospital in addition to providing a rapid response service to prevent unnecessary hospital attendance/admission.
The Children's Community Nursing Team provides nurse-led clinics to ensure that children and young people with non-acute conditions receive a service that is close to home.
You must be a registered children's nurse and have extensive clinical knowledge and skills in caring for children and young people with a variety of needs in acute services to enable you to practice autonomously and use your clinical decision making skills.
Experience of working within the community setting is desirable. You should have excellent inter-personal and communication skills and the ability to demonstrate sound theoretical evidence based knowledge.
You should be committed to the delivery of high standards of care, and have the ability to work both independently and as part of a multi-disciplinary team.
