A31 Primary Care Network (PCN) and Southern Health NHS Foundation Trust are excited to be jointly recruiting a Mental Health Practitioner work in our PCN's Mental Health Team. We welcome applications from any registered professional background where you have an interest in working within mental health services and have current UK registration.
A31 PCN comprises of 3 practices in north Hampshire, providing high quality and personalised care for patients, Alton Medical Centre, Chawton Park Surgery and Boundaries Surgery. Primary Care offers an excellent work life balance with Monday to Friday working patterns and no weekend or bank holiday work.
Main duties of the job
Working with our established Multidisciplinary Primary Care Mental Health team, this role is fundamental to the continued development and delivery of the PCNs primary mental health care offer. This team comprises staff employed by the PCN, Mind and Southern Health with wellbeing practitioners, peer support workers and social prescribers already in post and supported by other clinical members of the PCN and Southern Health including Mental Health GP lead, PCN Manager, Clinical Directors and Mental Health Primary Care team Manager for Mid and North Hampshire. The role provides clinical support to patients without our surgeries and will support with development of the service.
