Closer2Home is a specialist intensive home treatment service that supports young people with acute mental health problems in their homes by providing intensive input and support using a bespoke package of care tailored to each young persons needs.
The teams work in an integrated way, supporting one another to provide the service either remotely or sometimes by crossing borders into another trusts area. This means that a YP's needs are not restricted by boundaries, staff sickness has less of an impact on service delivery and YP receive a better service where ever they live. Closer2Home provides a service that helps young people in crisis to remain in their own home, in their own community where they can get involved in meaningful activities that are worthwhile to them.
Main duties of the job
Main Duties and tasks
* To support staff within a universal and targeted children's workforce.
* To work with senior colleague to develop skills in completing assessments, formulation of systemic treatment, reviews and be accountable for professional actions and policies.
* To participate in the development of a high quality, responsive service for children, young people, and their families/carers/friends whose presentation is complex and high risk.
* Assess children's individual care needs, develop, implement, and evaluate programmes of care to meet their mental health needs and improve quality of life through psychosocial interventions in line with NICE guidance.
* To offer consultation to colleagues across all teams within professional expertise, including representation in meetings with colleagues from both internal and external agencies.
* Under supervision of senior colleagues develop the skills to manage complex referrals and to assess the needs of the referred child or young person and to develop, implement and evaluate appropriate interventions, using professional expertise.
* To convene and lead on multi-agency meetings pertaining to a young person's care through the service.
About us
We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by the end of 2024.
Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.
The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.
Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on the day the trust is formed. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.
