An opportunity has arisen for an experienced Mental Health Nurse to join our multi-disciplinary Memory Assessment Service within the Older Peoples Service in the South Leeds area. You will have post registration experience and be able to demonstrate an in depth understanding of the needs of people with Memory problems and suspected dementia.

Main duties of the job

The role involves assessment of people over 65 who are presenting with cognitive impairment. You will work as part of an MDT to formulate diagnosis and provide both patients and Carers with advice and support post diagnostically. There is an expectation that you will be able to manage a varied caseload which will include completing initial and specialised assessments. Service User involvement is at the centre of our approach and you will embrace this in your practice. You will be able to demonstrate a range of therapeutic interventions and be committed to the concept of living well with dementia. Although working within a multidisciplinary team you will need to be able to work autonomously and flexibly.

You will have experience in a supervision role and be able to make a positive contribution to the development of the service. You will be encouraged to have access to regular supervision and to maintain your professional development. A good positive attitude towards this specialised client group needs to be demonstrated and also the aptitude to the contribution of continual development of the service incorporating evidence-based practice.

This role is community based and there is an expectation that you will provide senior nursing direction to junior staff and students across the South locality. It is essential that you have a full UK driving licence and use of a car.
