An exciting opportunity has arisen to join Brent Community Rehab's Discharge to Assess Team. We are looking for administrators, rehab assistants and physiotherapists to work closely with Brent Adult Social Care colleagues working with Reablement to deliver world class reablement to Brent residents. These roles are fixed term contract roles until 31 March 2025, with the possibility to extend beyond this date. This is a great opportunity to experience truly integrated working in a supportive environment to provide high quality patient centred care in the community.

Band 6 and 7 physiotherapy applicants should have a relevant professional qualification and clinical experience working in community rehab.

You will be provided learning opportunities and support to develop your skills e including clinical supervision, appraisals and training.

You will have access to the Trust's Academy and professional networks to access courses and meet your identified development needs.

Main duties of the job

Undertake specialised uni-professional and multi-professional assessments.

To develop and implement treatment plans for clients. These clients will present with primarily neurological conditions, but you may also see musculoskeletal conditions, osteoporosis, learning disability and/or mental health diagnosis.

Education/Qualification

Essential

  • Diploma/degree in Physiotherapy
  • Current HCPC registration
  • Evidence of up to date CPD portfolio
  • Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Neurology, Orthopaedics, Musculoskeletal Outpatients, Older People Rehabilitation


Desirable

  • Member of a Special Interest Group relevant to this post


Experience

Essential

  • Experience post registration as a Band 5 physiotherapist
  • Experience working at a junior level in a wide range of specialised clinical areas including outpatients, neurology, rehabilitation, orthopedics and respiratory


Desirable

  • Experience working with voluntary organisations.
  • Experience in service development projects


Skills and Knowledge

Essential

  • oSkills in the assessment and treatment of adults with complex multi-pathologies and neurological conditions
  • oKnowledge and understanding of complex medical conditions including neurological conditions, multipathologies and falls
  • oKnowledge of role of Allied Health Professionals and interdisciplinary team working
  • oAbility to apply core Physiotherapy skills across a range of clinical conditions within a client centered, goal setting
  • oKnowledge of outcome measures and evidence-based practice
  • oAbility to problem solve, prioritise and demonstrate clinical reasoning
  • oGood communication and report writing skills, in both verbal and written format
  • oBasic IT skills - Microsoft Word, Power point, Excel
  • oTeaching of junior staff / students, peers in Inservice training setting
  • oKnowledge and understanding of the role of Social Services and voluntary agencies
  • oInvolvement in Appraisal Scheme

  • oSupervision of assistant / junior staff
