An exciting opportunity has arisen to join the friendly and progressive Children's Home Acute Team (CHAT). The CHAT is an innovative nursing team, shortlisted for three final award Nursing Times Awards, that sits within an integrated child health team. The team delivers acute and ambulatory care at home to prevent hospital admission and facilitate early discharge.

Main duties of the job

We are looking for an experienced paediatric nurse who can work autonomously to care for acutely unwell children at home as well as been part of an ever-developing community nursing team. You will be responsible for delivering excellent acute nursing care in the community in and around Hampshire. You will be joining a high level of nursing expertise and be a role model to the nurses and students within the team. The post holder will have acute and preferably community experience.

This challenging and rewarding role offers the perfect opportunity to develop your acute skills whilst working both in the acute and community setting. You will have the opportunity to work as part of the CHAT developing existing ambulatory and unscheduled care pathways, preventing unnecessary admissions, avoiding hospital admissions and facilitating early discharge from the inpatient wards.
