About the role

The Deacon and ISS teams is a novel acute service pathway team that was designed to prevent admission to hospital and support people with learning disability in their own home. It was designed in 2016 on the principles of transforming care and to facilitate individual’s independence in the community. This role will work across both linked teams.

The Deacon unit is a 7 bedded inpatient unit for acute assessment and treatment for people with Intellectual Disability and Mental Health issues. The Intensive Support Service (ISS) team work to maintain individuals in the community and prevent admission. The team was first established in (add) and since then has led to a significant reduction in inpatient admissions.

The ISS role will work with CTPLD local consultant and the Transforming care group to identify and prevent admission of cases. The county wide role will work with others in the tam to support the individual I their home. Working collaboratively, prevention of

admission is the primary outcome. The role will work in conjunction with community teams to facilitate and agree an acute care management plan to prevent admission in times of crisis. Where needed, the role can also act as a gatekeeper to the inpatient unit.

The Deacon role will involve taking medical leadership for the team alongside the Service manager, Team manager, Senior Psychologist and Senior Nurse. These individuals make up the senior management team of both Deacon and ISS. The consultant in the post has responsible clinician status for the unit and will work alongside the management team to diagnose, formulate care plans, treat and facilitate discharge individuals passing through the unit.

The trust has a commitment to CPD and professional improvement. Subject to experience the individual will be encouraged to participate in management, leadership, research and service development. Specialist interest will also be encouraged where possible.

There is a on call rota of between 1:6-1:8 dependent on the number of permanent consultants.

About you

You will have:
  • A recognised basic Medical Degree and MR Psych or equivalent, Section 12 Approval and AC status.
  • Full Medical Registration (GMC) with a Licence to Practice
  • Be registered on the Specialist Register for Psychiatry of Learning Disability.
  • Approved Clinician Status and approval under Section 12 of the Mental Health Act
  • Preferably have management qualification or evidence of an interest in developing leadership skills
  • Research, study, audit and quality improvement are strongly advocated and SABP actively support your involvement. There is also access to the weekly trust wide academic programme.
  • A Clean driving licence with access to a car.

Key new developments over the past three years include:
  • The implementation of the Care Pathways – which focusses on providing specific interventions by cluster for the person’s recovery journey.
  • The implementation of GPiMHS – a newly funded service which provides a mental health service in primary care for those persons who do not require or meet the threshold of secondary care.
