This post will provide you an opportunity to work with people with Intellectual disability with commitment to collaborative working and actively supporting and liaising with other health and social professionals and agencies as well as have good relationships with mental health services. To assist patients in accessing mainstream primary care and acute secondary care services.
The role of Specialist community health team for LD is to offer secondary care interventions to people with an Intellectual Disability who are experiencing or causing in others high levels of distress. Intellectual disabilities (ID) (or learning disabilities or neurodevelopmental disorders) includes the presence of: a significantly reduced ability to understand new or complex information or to learn new skills (impaired intelligence) with a reduced ability to cope independently (impaired social functioning) which started before adulthood, with a lasting effect on development. This definition encompasses people with a broad range of disabilities. Others neurodevelopmental disorders such as Autism or ADHD, neuropsychiatric disorders such as Epilepsy and Dementia have high rates of co-morbidity with intellectual disabilities.

Main duties of the job
ID touches on many areas of medicine and presents a multi-faceted complex discipline, encompassing everything from clinical genetics to provision of adequate social supports. A structured and holistic approach is essential because of the complexity of needs (multiple physical, psychiatric, occupational, social, environmental, communication, educational problems) and comorbidities in many people with ID.
The post holder will be expected to work at a Trust grade level to support people with Intellectual disability for reviewing their mental health needs, reviewing medication, part of MDT meetings and attending team meetings (referrals, huddles and business meetings). The post holder will be working with the wider MDT and will be supported by psychiatry team that include one Consultant Nurse AC who also works across community and the ASU inpatient service, one nurse prescriber who is also an advance nurse practitioner and works into the community team. In addition there is one specialty doctor (current vacancy) and one full time Consultant Psychiatrist attached to the post who will be the supervisor for this post holder.
