The Calder Valley Enhanced Team is a Community Mental Health Team (CMHT) operating in Calderdale. As a community mental health practitioner you will be crucial to the MDT way of working and pivotal in delivering high quality, recovery focused care.
We are looking to recruit a skilled and experienced mental health practitioner who can work in a flexible and innovative way, providing assessments and interventions to address the needs of individuals accessing our service.
We have adopted the Flexible Assertive Community Treatment (FACT) model of care for service users who are presenting with increased risk and require a more intensive level of support. This feature allows professionals to involve the wider MDT in assessing and formulating daily risk management plans.
The team is made up of nurses, occupational therapists, support workers, psychologist and psychiatrist. We work extensively and collaboratively with other agencies for the well being of our service users.
Experience of working within the community with this service group would be advantageous although not essential. Mon - Fri 9am-5pm.
Full UK driving licence and use of own car is essential. Mileage is available to claim through the e-expenses system.
The organisation has well established clinical and professional networks. You will have access to individual clinical supervision and caseload management as well as team peer supervision.

Main duties of the job
There is access to a wide range of profession specific training and career progression opportunities available.
A key element of the role will be care coordination and leadership. Demonstrating skills, knowledge and experience to support colleagues with day- to day caseload management, duty worker role, clinical issues and supporting colleagues and teams in prioritisation with clinical acuity.
Undertake initial and risk assessments, formulate and review care plans of service users which promote engagement, stability recovery and risk management.
Manage a varied caseload and function as part of multidisciplinary team.
Work with service users with both short-term interventions and severe and enduring mental health needs to promote health and well being.
Document care delivery to a high standard in line with professional standards, local and national guidance.
Recognise situations that may be detrimental to the health and well being of the service user and initiates action that may be required.
Management of enquiries and direct referrals from other services such as SPA.
Support student education when allocated to the team and to contribute to the preceptorship and education of newly qualified staff where appropriate.
Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.

Responsible for acting up in the absence of the team leader.
Maintains accurate patient care records in accordance with professional codes of conduct and trust procedures and ensures records are made available when required by the appropriate agency.
Participates in the formulation of policies at unit level.
Maintains accurate mileage records.
Manages caseload with due economy of cost and time.
Ensures safe carriage, storage and administration of drugs prescribed (registered nurses only).
Responsible for the ordering of stock with due economy.
Produces monthly statistics in line with trust directives.
