The Patient Flow Team are looking for an enthusiastic and experienced nurse to join the existing team of In-Reach Practitionersthat support with assessing patients at the acute hospitals in Brighton and Haywards Heath, ascertaining their suitability for inpatient rehabilitation and supporting with the Discharge to Assess Model.
Main duties of the job
Full training will be provided, but experience in patient assessment and discharge pathways would be an advantage.
This role will require a driving licence and access to your own transport as you will be working across sites.

a) The In-Reach Practitioner will support the System Capacity and FlowManager in managing patient flow from acute providers to and across SCFTIntermediate Care Units.b) Ensure all bed capacity across the Trust is utilised whilst ensuringpatients experience safe, high quality care throughout their patientjourney.c) Work with system partners and colleagues to expedite safe transfersand provide safe alternative pathways for patients.d) To work collaboratively with external stakeholders and partnerorganisations to address urgent care local pressure, system surges andescalation ensuring capacity is fully utilised and patient flow ismaximised.
