The Acute Outreach team offer a responsive service to avoid unnecessary hospital admissions, other delayed transfers of care and facilitate hospital discharges. To support the care of service users and carers out of hours, to ensure timely interventions to support early treatment and reduce the risk of carer breakdown. The Acute Outreach team ensure an integrated approach to the care of people with dementia and functional mental health conditions. They work in partnership with local health, social care, and voluntary organisations.

You will work within the acute outreach team, where you'll contribute to delivering high-quality clinical services to service users and their families. Collaborate seamlessly with fellow providers, aligning with national strategies and NICE Guidelines. Engage in therapeutic interventions, following meticulous care plans, and making a positive impact on the lives of those navigating mental health-related difficulties.
Working with both Older adults and people with Dementia, you'll shape the well-being of the elderly population in South Staffordshire. Work in tandem with Staffordshire Police and Mental Health Services, embodying the Trust's commitment to people-centric care. Leverage your experience in older adult and dementia care, conducting assessments, formulating person-centred care plans, and managing risk assessments. Flexibility is your forte as you navigate unpredictable work patterns and client bases, offering compassionate care in highly distressing situations.

Main duties of the job
- To contribute to the assessment, planning, implementation and evaluation of the evidence-based interventions required, for a named service user/group under the guidance of a senior practitioner.
- To provide professional advice and support to other agencies and members of the multidisciplinary team.
- To be responsible for the appropriate clinical risk assessment of service users. Providing effective advice, guidance and decision-making process culminating in the coordination of any reviewed plan of care under the supervision of a senior practitioner.

- Evidence of professional updating and development within mental health services for older people
- Active and working on the NMC register
Experience

- Experience of delivering therapeutic interventions to older adults and their carers
- Experience of working with a range of statutory, non-statutory agencies
- Experience of service user and carer involvement working
Desirable
- Experience of working therapeutically in community settings
- Post registration experience in mental health
