Due to staff retirement, we are looking for a caring and compassionate, lead pharmacist to work within our expanding pharmacy team for our in-patient services at West Heath Hospital.
West Heath is a community hospital offering general medical and sub-acute care as well as specialist rehabilitation services for inpatients.

Main duties of the job
The post holder is professionally and managerially responsible for the provision and development of a professional and progressive pharmaceutical service in line with professional and government guidelines to the Wards at West Heat Hospital, with specific responsibility for West Heath Hospital Pharmacy Department and Service.

Job responsibilities
- Provision of a ward pharmacy service to all wards and supply of non-stock items including cytotoxic drugs. The service includes medication reconciliation, medication reviews with patients, falls reviews, reviewing every drug chart for correct dosages, drug interactions, side effects, legality and clarification of ambiguous prescriptions. Cancelling medication when the course is complete. Provision of medicines advice in specialised areas especially with regard to using medicines outside their product license and dose guidance in the elderly.
- Access summary care records and other patient records to ensure correct medicines information is obtained and patients are monitored as appropriate to their drug treatment
- Make records in patients notes relating to pharmaceutical care
- Planning for discharge
- Implement the Integrated Medicines Optimisation Committee (IMOC) Drug Formulary and review non-formulary medicines as appropriate. Support clinicians to make applications for drugs to be added to BCHC formulary where needed.
- Plan, manage and prioritise the pharmacy workload and help other pharmacy staff to prioritise their work.
- Plan and organise the provision of the pharmacy service to the Wards
- Plan pharmacy work rotas on a monthly basis with regard to skill mix to ensure that the Pharmacy service is adequately covered on all sites by the appropriate deployment of staff. This may include on call rotas if covering West Heath.
- Responsible for authorising time sheets and in house study leave.
- Chair regular departmental team meetings.
- Cascade Trust Team Talk
- Supervision and management of all pharmacy staff.
- Write, update and implement any pharmacy related drug policies, procedures and guidelines. Ensure these are reviewed and updated within necessary time period. Liaise with wards to ensure their adoption.
- Contribute to the Trust Medicines Optimisation policies and procedures, including those for pharmacy.
