Hello. Firstly, thank you for taking the time to look at this job. My name is Beth and I'm the team manager for the Core Team West in Wakefield. We have an appealing opportunity for a registered practitioner with mental health experience to join our service and work alongside our multidisciplinary team, as a band 6 practitioner. As a team we are friendly, open and supportive of one another.
We are looking for a motivated and friendly individual who puts service users first and in the centre of their care. We are looking for someone who will be open, honest, and transparent and someone who is ready for today, relevant for tomorrow and is willing to take on training opportunities to help develop and improve our service further. If this is you then you will be a perfect fit.
We work with services users aged 18-65 who have moderate to severe mental health issues. Referrals are made through several our trust services. Our base is a community team hub which helps to provide flexibility, openness and ease for support and advice. The base is within a central location with amenities, transport links and parking is easily accessible. Here's what some of the team says about the team-

Main duties of the job
The successful candidate will manage a caseload of service users, with a range of diagnoses and needs. They will be responsible for arranging care, liaison with other professionals and agencies as well as managing and reviewing care plans and risk assessments, ensuring these are done collaboratively and in a recovery focused manner. The successful candidate will also be able to offer evidence based clinical decisions and treatment options.
They will take responsibility as the duty worker on a rota basis, providing support to the wider core team caseload which may include medication queries, concerns from family members and generalised support. This may also include responding to crisis calls from the wider caseload when on the duty rota.
They will be able to offer support to other disciplines within the team and be confident in their decision making. The successful candidate should be able to support with and offer supervision to junior staff within the team.
The successful candidate will be an efficient organiser, managing their workload and be flexible to meet the needs of service.

JOB SUMMARY
Has continuing responsibility for the provision of skilled interventions to people with mental health problems in the community. Works within a multi-disciplinary team as a member of a Core CMHT.
Key Result Areas:
Professional:
Assesses needs and identifies problems relevant to the care of people referred. Devises a plan of care in partnership with the person and carers. Implements the planned programme of care to ensure a high standard is achieved. Reviews the effectiveness of the care provided and where appropriate initiates any action. Advises and supports the person and carers to promote health and well-being and to prevent illness. Recognises situations that may be detrimental to the health and well-being of the person and initiates action that may be required. Establishes and maintains satisfactory communication with all agencies involved in the persons care in both hospital and community. Functions as a member of a multi-disciplinary team.
