We are looking for a full time registered Mental health nurse/social worker or OT to join an established team that is based at Petersfield's Community Hospital. We are looking at recruiting due to the expansion of the CMHT.
Consideration will be given for the right candidate to be supported in development to Band 6 competencies level from Band 5 and we would welcome you to contact us to discuss.
The Bordon and Petersfield Community Mental Health Team operates from Petersfield hospital where this is our main site. The CMHT provides services to patients in the Bordon and Havant area as well . The team is well known for being approachable and open. We support each other and have developed a culture of honesty and integrity where problems are resolved quickly.
Our teams are working to make sure there is 'No Wrong Door' for our patients and service users. Our people say that being part of this work is hugely rewarding and energising, that they can work alongside partners to ensure people get the right care in the right place.
The Community Mental Health Team is focussed on delivering a recovery service for clients with severe and enduring mental health issues. We support our staff in developing their clinical skills in attending post graduate training such as physical health and nurse prescribing.
Due to the rural nature a car driver is essential with access to own car. We would consider the base being Havant with he expectation that you would travel to Petersfield and Bordon

The transformation will happen in staggered stages Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning in early May 2024. The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.

Main duties of the job
- To provide recovery based community mental health services within a multidisciplinary team to adult individuals with severe and enduring mental health needs referred to the Community Mental Health Team.
- This role, in collaboration with referred individuals and where appropriate family or carers, will coordinate and provide recovery focussed assessments, risk formulations, care planning, crisis and contingency planning, evidence based interventions, response, reviews, reports, discharge planning and attendance at appropriate meetings.
- Role further includes acting as clinical and case management supervisor and mentor.
- To make links with the Primary Health Care teams.
