The Oxford City & Northeast Oxon Adult Mental Health Team (AMHT) are advertising for three Band 7 senior care co-ordinator posts, one for each sub team. Each sub team is aligned with 2 or 3 GP PCN’s who they work closely with. The AMHT serves a multicultural population, including the two universities, for adults of working age (18-65), who are experiencing severe and enduring mental illnesses. The team covers the geographical area of the city of Oxford, the towns of Bicester and Woodstock, and the surrounding villages including Islip and Kidlington. We work in collaboration with partnership agencies such as Mind, Restore, Elmore and Response. This discipline of care gives an opportunity to visit patients in their own home or their supported living accommodation.

We use the principles of a flexible assertive community treatment (FACT) model of healthcare. We have incorporated FACT into the daily functioning of the team; which enables us to be focused on the recovery of the patient through a multidisciplinary (MDT) approach; with expertise from doctors, social workers, nurses, occupational therapists, support workers and psychological therapies staff. We also have twice weekly MDT meetings, where we discuss clinical matters within our supportive team. We strive to provide person centred evidence based care to patients, where possible working collaboratively with their families and carers.

The successful candidate will be a senior clinician within the team and lead a small team of newly qualified Nurses/Occupational Therapists, Nurse Associates, Peer Support Workers and Support Workers to provide care and interventions to a complex caseload of patients. The role be predominantly clinical, but also include some operational leadership and supervision to your small team of junior staff.
