We currently have a permanent vacancy for a full time experienced band 6 nurse to work in the Older Peoples Mental Health Intensive Home Treatment Team in Oldham.

Older Peoples Services are changing to meet the needs of people experiencing serious mental health problems in the community who are over 65 years of age and those people under 65 who or diagnosed with early onset dementia,, their partners, carers and families.

We are looking for an enthusiastic, driven nurse who can work autonomously in the community, complete full mental health assessments, manage risk in the community, while also being an integral part of our multidisciplinary team.

We are a small dedicated team of nurses, and support workers who work closely with our Consultant Psychiatrist, our colleagues in the community mental health team, care home liaison team, memory assessment team and our day hospital, Orchard House.

Main duties of the job

This role will involve nursing people with moderate to severe mental health problems at home as an alternative to admission to an acute hospital bed. In addition, this post will involve Gatekeeping admissions to inpatient services, as well as facilitating early discharge / leave pathways to support discharge from inpatient services.

The successful applicant will have sound knowledge and experience in risk assessment, formulation and management. Experience of working with individuals who present to mental health services in crisis and have symptoms of mental health illness. A knowledge and understanding of Mental Health Act '83 and the Mental Capacity Act are essential to this role. The successful candidate will receive regular supervision, an annual appraisal and excellent professional development opportunities within a supportive and friendly environment.

This is an exciting opportunity to join a small dedicated team of band 6 nurses and support workers who provide nursing care and treatment to patients living in the community.

We are fortunate to have our own offices in Forest house, we are also based with some of the other older peoples mental health service teams.

You will be given lots of support and opportunity to develop within the service.

This post will initially be based in Oldham but due to service changes will be moved to Middleton - no date as yet

Job description

Job responsibilities

the main duties of the role are

Triage referrals, visiting patients in their own homes or care homes.

Completing full mental health assessments, risk assessments and care plans.

Supporting students and TNA's

Assessing physical health and screening

Utilising Trust approved assessment tools.

Discussing plans with team and consultant psychiatrist.

Administering and monitoring medication in the community.

Preventing unnecessary admission to wards by providing intensive support at home.

Gatekeeping.

Attending ward rounds discharge planning meetings.

Safeguarding.

Providing education and support to patients and families in the community.

Working closely with the wider service, social care and other agencies.

Recording all activity on the PARIS system.

Liaising with GP's in the community.

Liaising with pharmacy, collecting and delivering medication.

Directing the work of the support workers in the team.

Engaging in weekly zoning meetings with team and managers.

Person Specification

Education and Qualifications

Essential

  • Good standard of General Education
  • RMN, OT or Social Worker
  • Current registration with professional body plus experience and short courses to post graduate diploma level
  • Completion of Preparation for Mentorship / Practice
  • Teacher / Fieldwork Educator qualification or willingness to complete


Desirable

  • Degree - or working towards completion
  • Non-medical prescribing qualification
  • Formal teaching qualification
  • Further training in brief psychosocial interventions


Experience

Essential

  • Significant post registration experience.
  • Proven experience of working with clients with complex needs.
  • Experience of working with individuals who present to mental health services in crisis and have symptoms of mental health illness
  • Experience of working in multi- disciplinary teams
  • Experience of providing supervision


Desirable

  • Adult acute mental health work experience
  • Experienced in the use of outcome measures such as CORE, PHQ9 and GAD7
  • Experience of working with a recovery model of care


Knowledge

Essential

  • Understanding of current mental health legislation, Safe Guarding and Child Protection Legislation
  • Awareness of NICE Guidance in the treatment of mental health problems and knowledge of treatments and interventions appropriate for individuals with mental health needs
  • Understanding of confidentiality and data protection issues.
  • Knowledge of psychosocial assessment and risk assessment / management processes


Skills and Abilities

Essential

  • Ability to undertake comprehensive mental health and risk assessments
  • Ability to construct effective packages of care and risk management plans
  • Ability to deliver a range of evidence based interventions
  • Excellent communication skills, both written and verbal.
  • Confidence to challenge decision making with senior colleagues in a constructive and pro-active manner
  • Comprehensive IT Skills
  • Ability to form productive working relationships with service users, carers and multi-disciplinary staff
  • Good organisational and prioritisation skills


Desirable

  • Skills in planning, delivery and evaluation of training.
  • Skills in the area of audit or research
