CREST is a new service within the mental health complex rehabilitation pathway specifically developed to provide different ways of working with individuals who have spent long periods of time in hospital. The service aims to provide psychologically informed, flexible and intensive packages of support focussing on building service user confidence to live a meaningful life in the community as an alternative to a hospital complex rehabilitation placement. The service also facilitates system change by providing a consistent approach to community rehabilitation and safety planning that supports more hospital providers to make discharge decisions.

The aim of the service is to provide psychologically informed, flexible and intensive packages of support aimed at building service user confidence to live a meaningful life in the community as an alternative to a hospital complex rehabilitation placement. The service will also facilitate system change by providing a consistent approach to community rehabilitation and safety planning that supports more hospital providers to make discharge decisions.

Main duties of the job

The role will focus on identifying, assessing, planning and carrying out interventions related to a service users rehabilitation needs. The successful applicant will also have a key role in the service evaluation.

The post holder will be responsible will work alongside the wider team in assessment barriers to discharge, supporting transitions to step down environments and continuing support in the community. They will have a good working knowledge of the principles of recovery and will practice clinically in a way that promotes therapeutic optimism and hope with service users.

We are looking to expand our team with individuals who share our passion for promoting recovery and supporting transition from locked rehabilitation environments into less restrictive environments. t. CREST work using a team approach to deliver care interventions which have been identified through the assessment process which is supported by a diverse multi-disciplinary team which includes peer support, medics, addictions specialist, family engagement worker, psychologists, occupational therapists and nurses. Due to the nature of the service user group there is a lot of multi-agency working involved in ensuring that high levels of care are delivered to service users on their recovery journey, and liaison with supported housing and other NHS and third sector organisations is something that the successful candidate will be required to undertake on a regular basis.

Job description Job responsibilities

You will be someone who can inspire others, create hope and make positive connections to help people build a meaningful life in the community. You will make a real difference to people placed in complex (locked) rehabilitation hospitals by providing an alternative offer from the new regional community team. You will be at the forefront of developing an exciting new regional community rehabilitation service with contemporary Occupational Therapy programmes to help people get home.

This means that supporting discharge from hospital requires flexible and intensive transitional community support closer to home, in the least restrictive environment, providing consistent, recovery focussed and contemporary interventions and integrated pathways of care. With funding from NHS England we are now in an exciting position to pilot a new intensive community complex rehabilitation service to work across Leeds, Calderdale and Kirklees/Huddersfield. It will offer a seven day a week, 8am until 8pm service. Being multi-disciplinary and multi-agency in nature it will create psychologically informed flexible care packages to support discharge home.

CREST was established as part of the complex rehabilitation project with the aim to support repatriation from locked rehabilitation placements with a strong focus on empowering service users and carers in being actively involved their recovery journey. The service operates 8am-8pm over a 7 day week to support individuals to prepare for, and fulfil a transition to a less restrictive environment working in a psychologically informed manner.

The Complex Rehabilitation project is part of the West Yorkshire Integrated Care System (WYICS) Programme for Mental Health and Learning Disability. Consultation with service users, carers, commissioners and mental health providers has highlighted the service user experience and challenges for people placed within mental health complex (locked) rehabilitation placements from West Yorkshire. Many people have experienced long lengths of stay in locked hospital placements, are disconnected from a community. Service users have shared very powerful experiences with us, describing a community model as not just about keeping me safe its about building a life worth living.
