This post supports delivery of the Patient Safety Strategy, PSIRF and LFPSE requirements, and also regulatory requirements, such as Duty of Candour.

The postholder is responsible for supporting delivery of the effective and timely delivery of the incident reporting and investigation process in the Trust, by working collaboratively and proactively to ensure thorough, proportionate investigations are conducted in accordance with Trust, and National policies and good practice.

The post sits within one of the teams accountable to the wider Quality and Safety Governance team and functions.

Under the direction of the Clinical Risk and Investigations Manager, the team will be required to manage their workload to ensure all timescales both internally and externally are met, and risks to deadlines are escalated appropriately and in a timely manner. The post holder should escalate any issues which may arise and potentially compromise timely completion or a change in scope of the investigation.

Main duties of the job

The post holder is responsible for ensuring adherence to systems and processes for the identification, appropriate investigation, reporting, and learning from incidents

Support the review of patient safety incidents to facilitate PSIRF panel workbook preparation, and post panel follow up.

Be responsible for updating actions and outcomes of incidents requiring further review, and ensuring timescales are adhered to for the management of reported incidents through the local incident reporting system (InPhase), including After Action Reviews (AAR), Multi-Disciplinary Team (MDT) reviews, case note reviews, timeline assessments and thematic reviews.

Provide a support package to the staff undertaking incident investigation and ensure that all safety improvement plans have had the appropriate governance and are generated in accordance with the format agreed by the Trust and commissioners.

Job responsibilities

Support the clinical service lines (CSLs) to ensure that incidents that have resulted in moderate and above harm to a patient, or meet the Trust or National Priorities, have a robust SMART action plan which is then monitored through internal governance processes.

Work with CSL teams to ensure that sharing and learning occurs regarding reportable events particularly where there are themes which can be shared which will improve safety across the organisation.

Support events that take place, and processes that are in place to facilitate the learning identified from patient safety incidents is shared across the organisation.

The post holder will support the Clinical Risk and Safety Lead and Clinical Risk and Investigations Manager with a caseload as part of an investigation team conducting reviews which may include a System Engineering Initiative for Patient Safety (SEIPS) approach into incidents reflecting the Trust and National priorities, in relation to PSIRF.

Person Specification

Qualifications

Essential

  • Educated to degree level or equivalent level of education or training in a relevant subject


Desirable

  • Registered Healthcare Practitioner
  • SEIPS/PSIRF training


Experience

Essential

  • Experience of working in a healthcare setting and experience of clinical risk management
  • Experience of investigating clinical incidents in a health care setting using SEIPS process
  • Knowledge of the Duty of Candour processes
  • Has an understanding of the audit and continuous improvement processes
  • Experience ensuring lessons learnt from patient safety incidents are turned into appropriate, realistic and achievable actions to reduce harm to patients / staff


Desirable

  • Experience working as a Registered Practitioner in a Healthcare setting which involves supporting staff
  • Experience of developing policies based on National frameworks / guidance
  • Experience of delivering and Evaluating training to a wide variety of Trust staff

  • Excellent organisational /prioritisation skills and the ability to meet deadlines and work under pressure
  • Self - motivated to be able to work on own initiative and prioritise workload effectively
  • Able to maintain composure when challenged and the ability to deal with distressing circumstances that may arise from conducting investigations
