Main duties of the job

CREST (community rehabilitation and enhanced support team) are expanding. We are looking for mental health practitioners for our developing Complex Emotional Needs (CEN) pathway to support individuals transitioning out of Level 2 in to the community. The role will involve coordinating the care of individuals in line with changes in national direction of the Care Programme Approach and offering evidence-based, trauma informed interventions to address core rehabilitation needs as well as manage risk within a community setting.

The CREST approach focuses on providing different ways of working with individuals who have spent long periods of time in hospital, to improve outcomes for this service user group. We strive to think creatively, and we are committed to continuous improvement, adapting to the needs of the population.

CREST-CEN is a new and developing pathway, working with service users who are experiencing complex emotional needs or interpersonal difficulties, which previously many have been referred to as a personality disorder pathway.

We are working towards providing a 7-day service, operating Monday-Friday 8am-8pm and 9am-5pm on weekends, to offer a more flexible approach.

This role is a great opportunity for registered professionals with an interest in working in rehabilitation and with people with complex presentations. We work with service users to plan and deliver interventions which focus on their rehabilitation needs and supports them working towards their recovery goals. We are a psychologically informed service which benefits from a diverse MDT to work to support individuals.

CREST support individuals within complex rehabilitation to address barriers to discharge, assess their rehabilitation needs and identify how these could be successfully supported in a least restrictive environment. The varied needs of the service users within CRESTs caseload and their long history of support within inpatient environments mean that CREST need to take an innovative approach to support to ensure successful outcomes.

The post holder will work closely alongside the existing and developing MDT within the service. In addition to being part of a supportive and dedicated team, there is also a strong focus on co-production and multi-agency working, with many of the caseload being supported by private hospital providers, supported living and a range of statutory and non-statutory organisations.

If successful, you will be responsible for a small caseload of service users who are supported by the wider multi-disciplinary team, and on a day-to-day basis, work within the framework of the team approach. You must have good working knowledge of the principles of recovery and work with service users in a way that promotes therapeutic optimism and hope.

Assessment will form a key part of CRESTs role, with referrals coming from case managers across the West Yorkshire region for individuals who are within, or on occasion being considered for, out of area rehabilitation with CRESTs focus being on identifying their current needs, barriers to establishing a discharge pathway and making recommendations on interventions that would support them in their discharge pathway.

You would also be required to coordinate the delivery of care within the role of duty worker, ensuring the effective prioritisation of care and resources. There will be the need for close liaison with service users, carers and loved ones, other service providers and the wider MDT to ensure high-quality care is delivered.

You will attend and participate in a number of MDT forums aimed at supporting the people we work with and supporting the team.

Travel will be required across the West Yorkshire Area. There will also be some travel to out of area hospitals outside of the West Yorkshire Footprint as part of assessment and engagement. Having a full driving licence with access to a car is essential - please confirm you have this on your application

CREST was established as part of the complex rehabilitation project with the aim to support repatriation from locked rehabilitation placements and a strong focus on empowering service users and carers in being actively involved their recovery journey.

The Complex Rehabilitation project is part of the West Yorkshire Integrated Care System (WYICS) Programme for Mental Health and Learning Disability. Consultation with service users, carers, commissioners and mental health providers has highlighted the service user experience and challenges for people placed within mental health complex (locked) rehabilitation placements from West Yorkshire. Many people have experienced long lengths of stay in locked hospital placements, are disconnected from a community. Service users have shared very powerful experiences with us, describing a community model as not just about keeping me safe its about building a life worth living.In order to meet the aims identified as part of the consultation project CREST has been working on developing a clinical model which supports individuals on their pathway from locked rehabilitation. This psychologically informed model focusses on placing the service user and their loved ones at the heart of what we do, assessing the need and identifying interventions that are needed to support and empower individuals on their recovery journey.
