The post holder will work as part of a multidisciplinary team, providing high quality, evidence-based care, and support children and young people with diabetes.
The post is based at King's Lynn but travel to external sites will be expected.
Your role will be to assess, plan, implement and evaluate high-quality holistic specialist diabetes nursing care under the guidance of the paediatric diabetes clinical nurse specialists.
Main duties of the job
To teach and assess families/carers to carry out specific diabetes cares.
To be responsible for planning, assessing, implementing and evaluating
individual children's care plans.
Work in collaboration with parents/carers to provide care to children
and young people in accordance with the child's plan of care to meet
their needs, and to liaise with all members of the multidisciplinary team
when changes occur.
To become an effective resource to the families/clinical teams.
