The Community Mental Health Team are seeking a skilled, motivated, organised and enthusiastic Mental Health Nurse to work within an established integrated, multi-disciplinary team, focusing upon individuals with severe and enduring mental health problems. The successful post holder will undertake responsibility for the care co-ordination and case management of a defined caseload of clients with complex and challenging needs as defined within Parts 2 and 3 of The Mental Health (Wales) Measure 2010.
Main duties of the job
To be responsible for the care co-ordination of a designated caseload of clients with severe and enduring mental health problems.
To ensure that the Care and Treatment Plans are adhered to and that standards are met in line with the Mental Health Measure ( Wales) 2010. To act as a point of contact for GP's and other interface services to offer advice support and signposting to appropriate agencies. Formulate and develop standard and complex outcome focused care and treatment plans. Ensure that a comprehensive risk assessment is completed and documented for actual and potential risk.
To be responsible for supporting clinical duties such as assisting with the running of the depot/clozapine clinic and the administration and supervision of long acting injections. Successful candidate to complete physical health assessment and side-effect monitoring for patients prescribed Clozapine.
To be responsible for assessing individuals in Crisis, completing comprehensive assessments of an individuals health and social care needs and agree the most appropriate course of action. To assess, evaluate and manage clinical risk. To formulate a plan of care and interventions based on identified need and deliver evidence based crisis interventions as appropriate. To work collaboratively with service users, carers and partner agencies in delivering high quality care.
The ability to speak Welsh is desirable for this post Welsh and/or English speakers are equally welcome to apply.
