We are looking for an experienced Community Registered Nurses with strong Leadership skills lead our forward thinking, Innovative Community Nursing Localities team.
The team is aligned with the five Portsmouth Primary care Networks in providing patients with a Multidisciplinary team approach for their health care needs.
The Localities service comprises of Nursing, Physiotherapy and Occupational Therapy staff. Also aligned and working closely with a Specialist Diabetes team and Enhanced Care Home Team.
The post holder will work within an integrated team to case manage co-ordinate and support patients with multiple complex long-term conditions. You will manage and coordinate a Nursing Team as part of the Community localities in Portsmouth.
Your Leadership as a Community Matron will include supporting and supervising your staff and enabling them, to provide high quality, person-centred care. Your excellent clinical, interpersonal and leadership skills will enable you to work collaboratively within, primary, secondary, and social care partners.
You will have or be working towards the extended and supplementary prescribing qualification.(V300)
Main duties of the job
As a Community Matron you will diagnose, prescribe, and implement a range of care interventions for people on your caseload, with an emphasis on avoiding unnecessary hospital admissions. Using your autonomous advanced clinical skills, you will plan care interventions for people with multiple, complex long-term conditions and coordinate and lead the team ensuring that patients receive timely and safe care. As caseload manager, they will have responsibility for ensuring all health and social care needs are met to improve health outcomes, functional status and quality of life. The post holder will work to ensure integration of all elements of care, increase patient choice and prevent unnecessary admissions to hospital and reduce the stay of necessary hospital admissions. You will work collaboratively with other appropriate services to ensure patients are appropriately stepped up and stepped down from the community matron caseload.
