We have an exciting opportunity for a Community Nursing Lead to join our established teams to work with our community nurses in the Integrated Community Teams.
Main duties of the job
The role is diverse, varied and practice focused, upholding the professional standards of practice, innovating service developments and working in collaboration with colleagues to create an outstanding service for the patients and families we work with. You will join an established, highly experienced peer group of Professionals within community nursing as well as physiotherapy and occupational therapy.

- Current registration with NMC / HCPC or ASC equivalent
- Educated to degree (level 6) (or diploma equivalent)
Desirable
- Extended skills qualifications relevant to the discipline (for example District Nurse SPQ)
- Hold a recognised teaching and assessment/ mentoring qualification
Experience
Essential
- Extensive post registration experience in the requisite discipline
- Breadth of clinical / professional experience which can be demonstrated in practice and support a range of patient / service user care requirements
- Experience of identifying and managing risk, planning appropriate interventions to mitigate identified risk, and working with escalation processes
- Experience of achieving positive outcomes for service users and the wider community through multi-disciplinary and partnership working and stakeholder engagement
