This post is fixed term for 6 months to meet service needs.
An exciting opportunity has arisen for the right person to join our team at Lewis Merthyr Day Unit, who who provide specialist dementia care for patients living in the community and support Tonteg Day Unit when needed
The post holder will be responsible for supporting the manager with the day to day running of the Day Unit. They will act as a role model and undertake day to day management of the nursing team, demonstrating clinical leadership.
The post holder will undertake comprehensive patient assessments, and facilitate the implementation and evaluation of nursing care to ensure the highest level of care is achieved and that the care is person centred, multi-disciplinary and evidence based.
The post holder will undertake assessments of patients both within the day unit and patients home via the open referral system. They will work closely with the client's carers and family, offering advice and support.
The post holder will participate in the weekly multi-disciplinary team meetings providing comprehensive reports on patients.

Main duties of the job
- Managing the Day Unit alongside the Day Unit Manager, providing effective day to day running of the Unit.
- Completing risk assessments for individual patients, monitoring and reviewing as required.
- Taking an active role in qualified and team meetings, taking the lead in the absence of the Day Unit Manager.
- Day to day management of a team of Day Unit Care staff, including qualified staff nurses, HCSW's, Local Authority including allocating duties/tasks as appropriate on a daily basis and undertaking annual PDR's for area of responsibility.
- Planning staff rotas, and co-ordinating with Local Authority to ensure sufficient staffing levels.
- Responding to patient and carer requests, queries or concerns in a timely manner.
- Assist the Day Unit Manager with the training requirements of the Day Unit - monitoring compliance and organising training when training needs are identified.
- Complete regular audits to monitor the effectiveness of practice and service delivery, and inform change as required.
