An exciting opportunity has arisen in the Intensive Home Treatment Team based at Lynfield Mount Hospital (Bradford)

We are looking for a qualified Community Mental Health Nurse/Occupational Therapist/Social Worker (Band 6) to join our busy team. (Occupational Therapists and Social Workers will be expected to work generically within the teams).

You will be responsible for the assessment, planning, implementation and evaluation of care to service users from the age of 16 yrs. You will contribute to the supervision and development of junior staff and regularly be Shift Co-Ordinator for the team. This includes, taking and making referrals to and from other health professionals and acute care services. The service is a 24 hour service and includes working nights.

You will have at least 2 years' experience as a registered mental health nurse working in a clinical setting. Ideally, working in an acute mental health setting. If interviewed, you should be able to demonstrate very good knowledge of relevant mental health practices specific to intensive home treatment team and all age adult mental health conditions. You should also be able to discuss guidance from NHS England / Department of Health forward planning for all age adult mental health service users.

Main duties of the job

To provide care and interventions, risk and safety management and safeguarding of all age adult patients involved with the Intensive Home Treatment Team (IHTT)

To engage in regular supervision with the Team Manager/Advanced Nurse Practitioner. Discuss, reflect and find solutions to challenges experienced in the role.

To deliver a high standard and quality service which enables patients to be supported throughout their care pathway whilst promoting independence and protecting choice, dignity, privacy and safety

To work in support of other allied health care professionals and services in the assessment, planning, and evaluation of care and treatment.

To communicate effectively with other services of the Care Trust, other health care professionals and with the patient family, carers/ visitors to ensure holistic care is delivered.

To supervise and delegate work to, Senior Healthcare Support Workers

To be Shift Co-ordinator in the team. Including, triage of referrals, inpatient services link worker and covering Bed Management

Person Specification

QUALIFICATIONS

Essential

  • Registered Nurse Mental Health
  • Evidence of continuing professional development.


Desirable

  • Clinical supervision qualification.
  • Post Graduate level education in psychological therapy.
  • Teaching (clinical practice) qualification/certificate or equivalent experience.


EXPERIENCE

Essential

  • 2 Years post qualification experience
  • Mental health care/treatment relevant to service.
  • Working with people presenting in mental health crisis.
  • Multi-agency working/working across service interfaces.
  • Working within a multicultural framework.


Desirable

  • Older adult age related illness and conditions
  • Experience of teaching/leadership of others.


KNOWLEDGE

Essential

  • Evidenced based clinical interventions relevant to the service.
  • Relevant specialist clinical courses/training.
  • Models of care and treatment relevant to working with people in mental health crisis
  • Evidence of continuing professional development.


Desirable

  • SystmOne PAS.


SKILLS

Essential

  • Assessing, formulating and working with people presenting in heightened state of distress.
  • Communicating (oral and written) complex/highly technical and clinically sensitive information to clients, their families, carers and other professional colleagues both within and outside the NHS.
  • Clinical risk assessment and contingency planning.
  • Able to meet the service needs for mobility across the geographical area covered


Desirable

  • Brief psychological interventions/strategies for managing crisis.
