Responsible for the specialist assessment, care and treatment planning for older adults who are experiencing mental health illness. To provide and monitor relevant treatment pathways, to improve their mental health and quality of life, in order to promote health, well-being and independence.
Main duties of the job
An opportunity has arisen for a Practitioner to join an Older Adults Community Mental Health Team.
If you come to work with us you will be a care coordinator in a team of dedicated and supportive professionals. We are a well established team who offer excellent care and treatment and we are looking for someone who has energy, enthusiasm, commitment, and who is committed to person-centred and holistic practice.
To be competent in completing specialist assessments, care and treatment plans and risk assessments.
To work proactively within a multi-disciplinary mental health service
To have a working knowledge of the Mental Health Act, Mental Capacity Act and the Mental Health Welsh Measure legislation.
To be motivated, organised and have the ability to work autonomously and within a team.
The ability to speak Welsh is desirable for this post English and/or Welsh speakers are equally welcome to apply.

- Significant post regestration experience within Old Age psychiatry
- Registered professional
- Skilled in risk assessment and risk management
- Understanding of the Mental Health Measure Wales
- Knowledge of evidence based interventions
- Up to date knowledge of assessment processes
- Awareness of clinical governance
- Understanding of contemporary approaches / developments in mental health
