One of the NHS Long Term Plan ambitions is to improve the experience of care for Young People aged 18-25.

Transition can be a difficult time for young people and their families, it is a lengthy process and involves various professionals and services. As part of the NHS Long Term Plan, Herefordshire and Worcestershire CAMHS are developing a multidisciplinary team consisting of Health staff and Professionally Qualified Youth Workers who are experts in supporting Young People’s personal, social, emotional and educational development.

We are looking to recruit JNC registered Youth Workers to engage and support young people who are receiving support from CAMHS to aide in the transitioning out of CAMHS and into Adult Mental Health Services/other services. This is an exciting opportunity to be part of a new and innovative service at the forefront of supporting children and young people within the local community.

The JNC recognises a range historic courses which will enable you to be registered with them, please see attached document in supporting information to view if your course is recognised by the JNC.

These are full time permanent posts, working 37.5 hours per week, Monday to Friday.

Main duties of the job

We require dedicated, experienced, and resourceful youth workers with a passion for supporting young people as they begin to navigate adulthood. You will ensure the young person is kept at the heart of the support you deliver whilst practicing in line with the national curriculum for youth work.

You will be working alongside CAMHS clinicians, AMHS clinicians, Community leaders and volunteers within the local community. You will be able to communicate effectively and advocate for the needs of the young people you are supporting whilst working as part of a multi-disciplinary team.

Job description Job responsibilities

Under the direction of the Team Manager or Mental Health Clinician, provide prompt assessment, engagement, signposting, onward referral and brief interventions for Young People transitioning out of CAMHS service. The post holder will offer advice, support and training to Primary Care, CAMHS, Adult Mental Health and the VCS whilst promoting recovery and social inclusion.

Support a Young Person’s social, emotional mental health through one to one case work.

Ensure young people have a voice and influence in the design and delivery of services in the district, neighbourhood and local community

Where referrals indicate, work jointly with the local mental health services and specialist mental health teams to support prompt assessment and where clinically appropriate, ensure the smooth transition of Young People to secondary care / specialist services, as well as towards community offers.

Following assessment, where indicated provide brief interventions using a casework approach, to support smooth transition of CAMHS and into other provisions which may not be health focussed.

Promote/deliver culturally capable, non-discriminatory enhanced mental health care services with an outcome orientated and recovery focus.

Person Specification Qualifications Essential

• Relevant professional qualification in Youth Work at degree level, recognised by the JNC
• Recent relevant evidence of continuing professional development

Skills & Abilities Essential

• Ability to communicate clearly and effectively in English in both verbal and written formats
• Skilled in the delivery of Youth Work for Young People aged 16 25
• Ability to work outside a Western medical model and embrace other ways of viewing individual's experiences
• Recovery orientated

Experience Essential

• Evidence of working for several years with Young People who have experienced mental health or emotional distress difficulties
• Assessment of Young People who require information, advice or guidance (IAG)
• Demonstrates high standards in written communication including being able to write clear reports and letters

Desirable

• Working in a primary care setting
• Experience post qualification working in a community Mental Health setting
• Worked in a service where agreed targets in place
• Facilitating/assisting in the running of groups

Knowledge Essential

• Knowledge of how poor mental health affects outcomes for Young People
• Knowledge of risks commonly presenting in Young People
• Demonstrates an understanding for the need to use Youth Work principles
• A robust understanding of the challenges that transition out of services pose to Young People
• Able to meet the travel requirements of the post
