We have new, exciting Transitional Care Senior Neonatal Staff Nurse/Specialist Midwife Band 6 opportunities within our 25 bed/cot Transitional Care unit, you will work as part of the Transitional Care team supporting families to care for their babies requiring more than normal care requirements but do not need to be admitted to the Neonatal Unit. This role will act as the link between Delivery suite, the Neonatal unit, Transitional care, and the Neonatal Community Outreach team. This is required to promote safe, early discharge of babies from Transitional care and prevent parent/infant separation and avoidable admission to the Neonatal Unit.

The focus of family integrated care is to keep babies and families together in order to enhance parent/infant bonding, reduce length of hospital stay and readmission rates all leading to improved chances of a healthier future for the whole family. We would like to hear from any experienced Neonatal Nurse or Midwife who feels excited by this opportunity.

Main duties of the job

Identifies a deteriorating baby and takes the appropriate action to manage the situation, liaising with the multidisciplinary team as appropriate.

To ensure the provision of excellent, evidence based care for infants and their families.

Offer professional advice and support to families whose babies require Transitional Care

Work in partnership with parents/carers to support and guide them in the care of their newborn infant promoting family centred and integrated care.

To enable parents to become partners in care for their babies. Supporting them with informed decision making processes.

To be a skilled supporter of infant feeding, providing excellent breastfeeding, expressing and responsive bottle feeding support to families.

To act as an advocate for the baby and family, providing them with information and support and directing them to the relevant support services where appropriate.

To start planning discharges from admission, liaising with the multidisciplinary team, and community Neonatal Outreach services as appropriate.

To maintain clinical skills and knowledge necessary to provide holistic, evidence-based nursing care. This includes expanded role responsibilities, following appropriate training.

Assist with Neonatal education opportunities for Transitional Care staff as an experienced Neonatal Nurse / Specialist Midwife working together with Neonatal clinical skills facilitator.

Communicate effectively, promoting open and trusting relationships.
