The post holder will be recruited as a Health & Justice Navigator primarily based at either Crawley or Worthing custody suite, with the flexibility of working across Sussex, dependent on skill and service need. The Post holder with be part of a team of professionals from different backgrounds, primarily completing screenings of individuals needs from within the custody environment.The post holder will use screening tools to identify health and social care needs. The post holder will assist individuals to navigate health and social care services to address identified needs and will be required to complete onward referrals where appropriate.The post holder will also be responsible for completing Court Reports outlining individual's needs that may need be taken into account when decisions about charging and sentencing are made.The post holder will work in a trauma informed way, providing appropriate liaison to prevent an individual having to re-tell their story.The post holder will require successful vetting from Sussex Police for this role.The post holder will be required to provide high quality information to key decision makers in youth and criminal justice agencies, including the police, courts, probation and Youth Offending Teams (YOTs).

Main duties of the job

**Please refer to the attached Job Description in summary:**SCREENING & ASSESSMENT1. Provide compassion to vulnerable individuals at points of acute vulnerability in the criminal justice process.2. To support criminal justice agencies to better identify vulnerabilities in their respective environments.3. Provide criminal justice agencies with advice and information regarding pathways to access further support.4. To secure referrals into mainstream health and social care services and other relevant interventions and support services from a range of statutory and voluntary providers.5. Completing the screening and management of service user need through initial information gathering, independent face to face screening demonstrating safe clinical decision making, seeking support from the H& J practitioner, identifying further assessment as indicated.6. Screening of the individual, through the relevant health databases in consultation with other service providers..This exciting service is currently funded by NHS England and is delivered by Midlands Partnership NHS Foundation Trust.The successful applicants must have knowledge of working with individuals who present with a range of vulnerabilities and identification of risks with the ability to follow direction and seek support where required.We are looking for a highly motivated individual and someone who is looking for a new challenge and have the skills to make a difference.

Key responsibilities within this role:1. Work within limitations and are of expertise.2. To seek support and guidance from H& J Team Lead where necessary.3. Provide an embedded service in police custody suites, working within legislation requirements, such as PACE (1984).4. Work as part of a team with the freedom to work independently, particularly in relation to providing advice to service users, carers, and professionals who contact the service.5. Liaise with other agencies as required dependant on the individuals presentation, work closely with other agencies, such Custody Health Care Professionals, and community agencies.6. Be accountable for aspects of own clinical practice. 7. Ensure that those aspects of service, delivered as part of remit, meet local quality standards.8. Ensure that the dignity and respect of individuals is maintained.9. To maintain effective communication between health, social care and criminal justice agencies, individuals, relatives, carers and the multi-disciplinary team, enabling better identification and responses to provide effective outcomes.10. Communicating and recording information accurately, sensitively and appropriately for use by criminal justice agencies (i.e. police systems) to inform the decision making process.11. Promoting effective communication with people who are troubled or distressed.12. Working collaboratively across teams, agencies and organisational boundaries to ensure the delivery of an integrated service through effective communication.13. Maintaining excellent communication with individuals regarding their health and social care needs liaising with carers and professionals as required.14. Actively seek support and supervision to maintain safe practice and facilitate robust decision making.

Person Specification

Qualifications

Essential

  • Professional recognised qualification such as RMN, RGN, Learning Disability Nurse, Social Worker, Occupational Therapist, AHP.


Desirable

  • Degree qualification in relevant subject or demonstrable evidence or equivalent experience relevant to field of practice.


Skills, Knowledge & Abilities

Essential

  • Knowledge of mental health issues and/or learning disability. Knowledge of Criminal Justice System, including roles and responsibilities of staff working in it. Knowledge of use and interpretation of health screening tools. Knowledge of assessing and mitigating risk - including risk of harm to self and others. Knowledge of a wide range of vulnerabilities including the needs of children and young people. Knowledge and understanding of capacity and consent. Knowledge of theoretical approaches to working with vulnerable people. Knowledge of appropriate health and social care interventions. Knowledge of relevant law, as a minimum Mental Health Act, Mental Capacity Act, Children Act, Social Care Act and PACE code C. Knowledge of human growth and development. Knowledge of Safeguarding Adults and Children Knowledge of Information Governance, Data Protection and GDPR.


Desirable

  • Specialist knowledge in substance misuse. Specialist knowledge in Autism and ADHD. Specialist knowledge in Personality Disorder Specialist knowledge Domestic Violence. Knowledge of Psychosocial and Psychological Approach.
